Correlation between Interleukin-6 and fatigue in patients with non-small cell lung cancer: a longitudinal study (English)
Abstract [en]
Cancer-related fatigue is a common and often debilitating symptom among patients with non-small cell lung cancer (NSCLC), affecting both quality of life and treatment outcomes. The cytokine Interleukin-6 (IL-6) is a central mediator of the inflammatory response and has previously been associated with fatigue and other systemic cancer-related symptoms. A better understanding of the relationship between IL-6 and fatigue may contribute to the development of more targeted supportive and therapeutic interventions. The aim of this study was therefore to investigate the relationship between serum IL-6 concentrations and perceived fatigue in patients with NSCLC during ongoing treatment. Fatigue was assessed using a shortened version of the Multidimensional Fatigue Inventory (MFI), consisting of five selected questions related to fatigue. Responses were graded from 1 (“yes, that is true”) to 5 (“no, that is not true”), where higher scores indicated higher levels of fatigue. Serum IL-6 concentrations were measured, and the relationship between IL-6 levels and MFI scores was analyzed using correlation analysis. The results showed a weak positive correlation between IL-6 and fatigue. A regression analysis of the combined data produced a coefficient of determination of R² = 0.0554, indicating that only 5.54% of the variation in IL-6 levels could be explained by variation in fatigue scores. Although the positive correlation suggests a possible association between increased IL-6 and fatigue, the relationship was weak. This indicates that several other biological, psychological, or treatment-related factors are likely to contribute to the experience of fatigue. In conclusion, the findings suggest that IL-6 only partially explains cancer-related fatigue in patients with NSCLC. These results may contribute to a better understanding of the complex mechanisms underlying this symptom and provide a basis for future research on individualized treatment strategies.

Cancerrelaterad fatigue är ett vanligt och ofta försvagande symtom hos patienter med icke småcellig lungcancer (NSCLC), vilket påverkar både livskvalitet och behandlingsutfall. Cytokinen Interleukin-6 (IL-6) är en central mediator i den inflammatoriska responsen och har tidigare kopplats till trötthet och andra systemiska symtom vid cancer. En ökad förståelse av sambandet mellan IL-6 och fatigue kan bidra till utvecklingen av mer riktade stöd- och behandlingsinsatser. Syftet med denna studie var därför att undersöka relationen mellan IL-6koncentration i serum och upplevd fatigue hos patienter med NSCLC under pågående behandling. Fatigue bedömdes med en förkortad version av Multidimensional Fatigue Inventory (MFI), bestående av fem utvalda frågor relaterade till trötthet. Enkätsvaren graderades från 1 (“stämmer helt”) till 5 (“stämmer inte”), där högre värden motsvarade högre grad av fatigue. IL-6-koncentrationer mättes i serum, och sambandet mellan IL-6-nivåer och MFI-poäng analyserades med hjälp av korrelationsanalys. Resultaten visade en svagt positiv korrelation mellan IL-6 och fatigue. En regressionsanalys av den sammanställda datan visade ett determinationsvärde på R² = 0,0554, vilket innebär att endast 5,54 % av variationen i IL-6 nivåer kan förklaras av variationen i fatigue poängen.
