Abstract [sv]

Det finns ett behov av att utforska patienters subjektiva upplevelser av en behandling för att få kunskap om hur en behandling kan utvecklas. Rumineringsfokuserad kognitiv beteendeterapi (RFKBT) i grupp är en transdiagnostisk behandling som i en tidigare studie visat lovande resultat för att behandla insomni. Då drygt tio procent av vuxna i Sverige lider av insomni, vilket inverkar negativt på välbefinnande och funktionsförmåga, finns det anledning att utforska behandlingar som kan gynna denna patientgrupp. Vidare är det intressant att utforska hur gruppbehandling kan nyttjas då det är resurseffektivt, speciellt inom primärvården. Syftet med denna uppsats var därmed att undersöka hur deltagare med insomni, som genomgått RFKBT i grupp, upplevde gruppbehandlingen. Nio deltagare, i två behandlingsgrupper intervjuades och data analyserades kvalitativt genom tematisk analys. Analysen resulterade i fyra huvudteman: gruppens betydelse för behandlingsutfall, tilltro till den egna förmågan, strukturerat upplägg med utrymme för flexibilitet samt förändrat mående i positiv riktning. Resultatet visade att ett gott gruppklimat bidrog till social gemenskap och nya perspektiv. Huruvida sömnen upplevdes förbättrad varierade mellan deltagarna. Däremot upplevde samtliga minskad oro och ältande, specifikt kring sömnen. Ett annat förhållningssätt till den egna problematiken lyftes, där sömnbesvär inte längre upplevdes skapa samma obehag. Tack vare att ha fått strategier, verktyg och nya förhållningssätt upplevde deltagarna en ökad tilltro till den egna förmågan där de hade makt att förändra det egna måendet. Sammantaget bidrar uppsatsen till en fördjupad kunskap om deltagarnas upplevelser av behandlingen, där gruppen tillsammans med behandlingskomponenterna skapade en upplevelse av self-efficacy, ökad agens, normalisering, självmedvetenhet och ett förändrat förhållningssätt till sin problematik. 

Abstract [en]

There is a need to explore patients' subjective experiences of a treatment in order to acquire knowledge towards the development of a treatment. Rumination-focused cognitive behavioural therapy (RFCBT) in groups is a transdiagnostic treatment that in a previous study, showed promising results for treating insomnia. As approximately ten percent of adults in Sweden suffer from insomnia which has a negative impact on well-being and functional ability, there is therefore a need to explore treatments that can benefit this patient group. Furthermore, it is interesting to explore how group treatment can be used as it is resource efficient, especially in primary care. The purpose of this essay was to investigate how participants with insomnia, that had undergone RFCBT in a group, experienced the group treatment. Interviews were undertaken with nine participants in two treatment groups, and the data analysed qualitatively through thematic analysis. The analysis resulted in four main themes: the group’s importance towards the outcomes of treatment, confidence in one's own ability, structured arrangement with room for flexibility and changes in moods in a positive direction. The results showed that a good group climate contributed to social community and new perspectives. Whether sleep was perceived to be improved varied between participants. On the other hand, all experienced reduced worry and kneading, specifically around sleep. A different approach to one's own problems was raised, where sleep problems were no longer perceived to create the same discomfort. Thanks to having received strategies, tools and new approaches, the participants experienced an increased confidence in their own ability where they had the power to change their own mood. Overall, the essay contributes to a deeper knowledge of the participants' experiences of the treatment, where the group together with the components in treatment created an experience of self-efficacy, increased agency, normalization, self-awareness and a changed approach to their problems.
