    Abstract [en]

    Loco-regional recurrence following resection of squamous cell carcinoma of the upper aero-digestive tract is a major problem. Cells exfoliated from malignant tumours have been considered capable of implanting and giving rise to local recurrence or metastatic deposits. A prospective study was performed examining washings from the gloves and instruments used in major head and neck operations to determine if exfoliated cells were indeed present. Following removal of the main tumour specimen all surgeons washed their hands in a solution containing a cell fixative and all instruments were soaked in the solution. Samples were then centrifuged and stained using a modified PAP technique. On occasions nuclear fragments consistent with squamous cell carcinoma were present in both glove and instrument washings. We recommend that all surgeons change gloves and instruments following removal of the main tumour specimen and prior to irrigation of the operative field with a tumoricidal agent.

    Abstract [en]

    This paper examines user requirements for a new audiology information system for use in audiology departments by audiology staff. There is a lack of literature on requirements for an audiology information system. This ethnographic study describes the current audiology service and the equipment used in the diagnosis and treatment of hearing loss. Eliciting user requirements from all the users of a clinical information system provides the best possible outcomes for all user groups: clinical, administration and hearing aid technicians. The study includes a review of audiology internal policies and a survey of all potential end users of a new audiology information system in the Health Service Executive (HSE) community service. The findings identify the six most important user requirements in a new audiology information system. These priority requirements include the storage of all Noah files, multiple patient search options, have all test results stored in the patient's electronic record, be capable of generating IT-based reports and store the audiogram in the patient's electronic record (EHR). Other findings include the desire for a paperless system and for the integration of all audiology equipment and existing IT databases, with up-to-date? information system involving automated processes.

    Limiting the spread and effect of infections in hospital populations is a priority of healthcare providers. Approximately 1 in 20 in-patients develop a hospital acquired infection (HAI) in Ireland, and the rising prevalence of antibiotic-resistance strains forces a shift in focus from a curative to preventative approach to management.

    Addressing issues of infection control in hospital, this paper reviews the potential for radio frequency identification (RFID) to positively impact infection control. A retrospective analysis of patients’ movements through an interventional radiology department (IR) in a busy teaching hospital was conducted to examine infection and cross-infection potential in the course of routine working.

    A literature review was conducted and observations were recorded over a 4-week period. The number of infected patients was noted, along with time spent in establishing if an infection was present.

    Thirty-four of the 135 patients who visited IR were infected; MRSA infection was most common. The overall theme to emerge was that while information was available it was fragmented and not easily accessed which makes tracking the movement of infections throughout a hospital difficult.

    Potentially RFID can assist in limiting cross-contamination opportunities. Because RFID technology has the ability to establish an auditable list of contacts between staff and patients, a patient’s journey throughout an organisation can be retrospectively analysed. This information provided can then be used to identify other patients at risk and better inform the use of antibiotics. This ubiquitous approach also reduces time spent tracking infectious incidences.

    ”Mechanical Debridement –An ergonomically designed hand tool for wound cleansing” is a final thesis that has been executed independently, during spring semester 2012, by Mikael Johansson in a close collaboration with the other group member Fredrick Ekman. The thesis (22.5 ECTS credits) has been executed as course in the Innovation and Design Engineering program within the Faculty of Technology and Science at Karlstad University.

    The project was carried out as a student project commissioned by Medilink AB which is a small company that manufactures and sells medical equipment. The company has got indications from professionals within the healthcare sector that there is a need for mechanical wound cleansing systems, which could simplify the wound cleansing process.

    A preliminary study, with focus on wound treatment and wound cleansing, was carried out collectively by the group to get a basic understanding of the subject and to find possible problematic areas. In the subsequent research phase the individual part of the project begun, which has a focus on ergonomics and medical equipment. This was conducted with interviews, literature studies and field studies.

    Several different idea generation methods were used to generate many ideas that were reworked into five concrete concepts. In consultation with the supervisor these concepts where evaluated with different concept evaluation methods to choose the most relevant for this project. To further evolve and adapt the concept for the target group, a workshop with nursing students were conducted. This lead to the final concept, an ergonomic, pen-shaped wound cleansing tool.

    In the final phase the chosen concept were developed further by a CAD-model and a physical model to visualize the concept. This physical model was also tested through a validation workshop in order to get feedback regarding the design.

    This study of collaboration among a multidisciplinary team of healthcare workers demonstrates that elements intrinsic to the interaction constitute a delicate ecosystem. As the balance between actors, digital media and paper artefacts fluctuates, so too the nature of the interaction and collaboration changes. Intrinsic to the multidisciplinary team (MDT) ecosystem is specialist knowledge, radiological images, pathology samples, together with the interpretation of the patient’s findings, as well as the roles and responsibilities of the active participants and observer collaborators.

    Identifying an appropriate method to evaluate the use of technology during patient case discussions at multidis- ciplinary medical team (MDT) meetings, is problematic. A number of approaches conducted over an extended pe- riod of study are described and the lessons learned are explained. A framework is proposed to serve as a basis for evaluation of technology use in these complex collab- orative work settings that incorporates hospital, technology and people perspectives.

    The modern medical industry successfully utilizes Laser Powder Bed Fusion (LPBF) to manufacture complex custom implants. Ti6Al4V is one of the most commonly used biocompatible alloys. In surgery practice, infection at the bone-implant interface is one of the key reasons for implant failure. Therefore, advanced implants with biocompatibility and antibacterial properties are required. Modification of Ti alloy with Cu, which in small concentrations is a proven non-toxic antibacterial agent, is an attractive way to manufacture implants with embedded antibacterial functionality. The possibility of achieving alloying in situ, during manufacturing, is a unique option of the LPBF technology. It provides unique opportunities to manufacture customized implant shapes and design new alloys. Nevertheless, optimal process parameters need to be established for the in situ alloyed materials to form dense parts with required mechanical properties. This research is dedicated to an investigation of Ti6Al4V (ELI)-1 at % Cu material, manufactured by LPBF from a mixture of Ti6Al4V (ELI) and pure Cu powders. The effect of process parameters on surface roughness, chemical composition and distribution of Cu was investigated. Chemical homogeneity was discussed in relation to differences in the viscosity and density of molten Cu and Ti6Al4V. Microstructure, mechanical properties, and fracture behavior of as-built 3D samples were analyzed and discussed. Pilot antibacterial functionalization testing of Ti6Al4V (ELI) in situ alloyed with 1 at % Cu showed promising results and notable reduction in the growth of pure cultures of Escherichia coli and Staphylococcus aureus.

    We revisit a series of studies on the work of multidis- ciplinary medical teams with a view to identifying op- portunities for the use of intelligent systems to support their complex cooperative work, and the challenges that might arise in developing such systems. We focus spe- cially on the activities performed during the multidisci- plinary medical team meeting (MDTM) and review the literature on MDTMs, as well as our own longitudinal analysis of several MDTs in a large teaching hospital over a period of ten years.

    We investigated lung pathology in 61 HIV-positive patients who underwent 79 bronchoscopies. There were 27 cases of pneumo cystis carinii pneumonia (PCP), 22 cases of pyogenic pulmonary infection and six cases of mycobacterial infection. Eleven cases of non-specific interstitial pneumonitis were found and 10 intravenous drug abusers had birefringent pulmonary talc granulomata in biopsy specimens. Women and intravenous drug abusers had higher rates of pyogenic pulmonary infection than men and other risk groups respectively. When compared with a previous survey from the same hospital using the same methodology the present study showed a change in the risk groups with 50 % more intravenous drug abusers, 42% more haemophiliacs and 45% fewer patients with sexually transmitted infection. However, apparent changes in the spectrum of diseases encountered were not significant.

    AbstractThis paper presents a novel micro-fabrication method using focused ultraviolet (UV) light to manufacture three-dimensional sawtooth structures in ultra-thick negative photoresist to fabricate a novel multi-prism x-ray lens. The method uses a lens to shape the UV beam instead of the photomask conventionally used in UV lithography. Benefits of this method include the ability to manufacture sawtooth structures in free form, for example in circular shapes as well as arrays of these shapes, and in resist that is up to 76 μm thick.To verify the method, initially a simple simulation based on Fourier optics was done to predict the exposure energy distribution in the photoresist. Furthermore, circular sawtooth gratings were manufactured in a 76 μm SU-8 resist. The UV lens was fabricated using electron beam lithography and then used to expose the SU-8 with UV light. This paper details the complete developed process, including pre-exposure with an e-beam and cold development, which creates stable sawtooth structures. The measured profile was compared to the ideal sawtooth and the simulation. The main discrepancy was in the smallest feature size, the sawtooth tips, which were wider than the desired structures, as would be expected by simulation.

    The NanoXCT project aims at developing a laboratory nano-CT system for non-destructive testing applications in the micro- and nano-technology sector. The system concept omits the use of X-ray optics, to be able to provide up to 1 mm FOV (at 285 nm voxel size) and down to 50 nm voxel size (at 0.175 mm FOV) while preserving the flexibility of state-of-the-art micro-CT systems. Within the project a suitable X-ray source, detector and manipulation system are being developed. To cover the demand for elemental analysis, the project will additionally include X-ray spectroscopic techniques. These will be reported elsewhere while this paper is focused on the imaging part of the project. We introduce the system concept including design goals and constraints, and the individual components. We present the current state of the prototype development including first results.

    The NanoXCT EU FP7 project [1] aims at developing a laboratory, i.e. bench top sized X-ray nano-CT system with a large field-of-view (FOV) for non-destructive testing needs in the micro- and nano-technology sector. The targeted voxel size is 50 nm at 0.175 mm FOV, the maximum FOV is 1 mm at 285 nm voxel size. Within the project a suitable X-ray source, detector and manipulation system have been developed. The system concept [2] omits the use of X-ray optics, to be able to provide a large FOV of up to 1 mm and to preserve the flexibility of state-of-the-art micro-CT systems. The targeted resolution will be reached via direct geometric magnification made possible by the development of a specialized high-flux nano-focus transmission X-ray tube. The end-user's demand for elemental analysis will be covered by energy-resolved measurement techniques, in particular a K-edge imaging method. Timepix [3] modules were chosen as the basis for the detector system, since a photon counting detector is advantageous for the long exposure times that come with very small focal spot sizes. Additional advantages are the small pixel size and adjustable energy threshold. To fulfill the requirements on field-of-view, a detector width > 3000 pixels was needed. The NanoXCT detector consists of four Hexa modules with 500 mu m silicon sensors supplied by X-ray Imaging Europe. An adapter board was developed to connect all four modules to one Fitpix3 readout. The final detector has an active area of 3072 x 512 pixels or approximately 17 x 3 cm(2). In this contribution we present the development of the Timepix based NanoXCT detector, it's application in the NanoXCT project for CT and material specific measurements and the current status of results.

    Protecting newborn babies from abduction is a major concern for maternity care providers. Three-hundred abductions were reported between 1983 and 2014 in USA [1]. Electronic infant tagging systems support infant security through monitoring, and surveillance. Despite the high cost of implementation and maintenance, this highly sensi- tive protection system generates false alarms, adding an unnecessary burden on healthcare professionals. Few studies so far examined in detail the problems associated with the false alarms generated by the infant protective system. In this study, we report an analysis of dif- ferent types of alarms generated, and issues associated with an infant tagging system in a major Maternity Hospital setting in the Republic of Ireland. With Hospital permission, false infant tags alarms were observed and recorded over 10 consecutive days. Midwives were asked their recommendation for improvement, and an economic analysis of the impact of false alarms was undertaken. A descriptive statistical analysis is used to examine different alarms. 13.4 h of infant tag alarms were generated in 279 occasions over the 10 days. Frequent false alarms contribute to alarm fatigue and mistrust of the infant tagging system. A well-managed infant tagging system could save €10,944 annually on one ward. This study prompted service improvement initiatives that included training, and technological modification, which reduced the false alarm rate.

    A survey of health information seeking behaviour among young adults, 18-29 years old, reports that as well as searching for information for their own use, searching for information to help family and friends is also practised?. Overall, the top 5 categories of information searched are 'Personal Health', 'Symptoms', 'Healthy Diet', 'Mental Health' and 'Treatment'. For the Top 5 Information searches for Self, 'Disease Prevention' is in 5th place and displaces 'Mental Health'. Respondents mostly report a high regard for the information they retrieve in their searches. The use of support groups for themselves is reported, as well an altruistic motivation to support others. While the motivation for many searches is to avoid attending a health professional, approximately one-third search after their visit. Of the respondents who are Swedish residents, 42% are unaware of the availability of their Journalen, online health record in Sweden, and of those 56.7% who know of it, 18% have never accessed it.
