    Abstract [en]

    Rationale, aim and objectives: Patients experiencing high care quality and satisfied patients are more likely to follow treatments. Patient satisfaction is an important contributor to physical and mental health-related quality of life. Research emphasises the need to further study satisfaction from the patients’ perspective. The aim was to describe patients’ experiences of care quality and the relation to their satisfaction during hospital stay. 

    Methods: A qualitative descriptive design was used. Interviews were conducted with 22 patients discharged from hospital. Data was analysed by conventional content analysis.

    Results: Four categories and seven subcategories describing patients’ experiences of care quality and the relation to their satisfaction emerged. Desire to regain health comprised waiting for treatment, being cured, having hopes of being cured and described the treatment and health outcome of hospitalisation. Need to be met in a professional way as a unique person comprised receiving personalized knowledge, receiving healthcare by competent healthcare personnel and described the way patients need to be met by healthcare personnel. Need to be involved comprised taking responsibility for own health, leaving responsibility for own health and concerned the patients’ way of handling hospitalisation. Need to have balance between privacy and companionship concerned the relationship to fellow patients.

    Conclusions: Health condition is of great importance to patients’ experiences of quality of care and their satisfaction in relation to hospital stay. The healthcare personnel need to be aware that seriously ill patients may never be completely satisfied. Furthermore, healthcare personnel must do their utmost to provide the patients with person-centered care.  Hospital managers must consider the design of wards with respect to such matters as multiple-bed versus single-bed rooms and heads of nursing must carefully plan each patient’s accommodation.

    Abstract [en]

    Outdoor education is becoming an increasingly larger subject in Swedish schools.  It makes up about a third of the curriculum in Physical education. In this quantitative study, the result shows that physical education teachers believe they have enough material resources to teach in outdoor education. However, this not applies to the students and the material they bring from home. The teachers feel that they have enough knowledge about outdoor education to teach in the subject and most considers that they have an area close to the school there they have opportunity to teach outdoor education. When it comes to how the school and colleagues are working with or against the physical education teachers, the teachers responded that a small majority of the schools work together with success regarding the curriculums in outdoor education. When it comes to how involved their colleagues are in planning and implementing outdoor activates the majority answered that they receive help from their colleagues.

    Abstract [en]

    Background: Persons with dementia diseases are today a large group that will increase significantly over the next few years. Behavioral and Psychological Symptoms of Dementia (BPSD) are very common and can cause major suffering. For dementia care professionals, persons with dementia suffering from BPSD may induce stress in the caregiving situation. With proper training and support, this stress may be reduced. In the BPSD register, the dementia care professionals may get information, advice and help suggesting non-pharmacological interventions that can help the person with dementia. Aim: The aim of the study was to investigate caregivers opportunities and experiences in implementing BPSD registration and follow up of non-pharmacological interventions in the caregiving situation within residential care for dementia. Furthermore, the aim was to compare if the dementia caregivers experience had significance for the experienced value of BPSD registration in Behavioral and Psychological Symptoms of Dementia. Method: A quantitative survey was conducted on residential care for dementia, N=5. The study- specific questionnaire covered possibility for BPSD- registration and interventions, considered value of BPSD estimation, and if non-pharmacological interventions are performed followed up, and considered to reduce BPSD. The results are presented using tables and diagrams. Kruskal Wallis test was used to compare variables between subgroups with varying working experience. Result: Most dementia caregivers considered that they had possibility to conduct BPSD registration 50% (n=28). They expressed 88% (n=49) that the non-pharmacological interventions that were initiated and performed decreased BPSD. Further they experienced BPSD follows up being important, 100% (n=56). There were no statistically significant differences between number of professional years and the compared variables (p-values ranged 0,266-0,981). Conclusion:The study found that dementia care professionals considered having possibilities to perform BPSD estimation, non-pharmacological interventions and follow- up. BPSD registration was experienced to be a helping tool for the dementia providers; and the length of the working experience had no significant relation to how they graded the value of the BPSD estimation.

    Syftet med studien var att undersöka vårdnadshavares och förskolepersonals syn på socker i förskolan. I studien användes en enkätundersökning som riktade sig till vårdnadshavare med barn i förskola och förskolepersonal. Respondenterna nåddes genom Facebook-grupper. I studien deltog 372 deltagare, 173 av respondenterna var vårdnadshavare till barn i förskola och 199 var förskolepersonal. Resultaten från enkäten ämnad för vårdnadshavare visade att de hade en generellt neutral till negativ attityd till socker. Vårdnadshavare tyckte att humöret hos deras barn var det som påverkades mest efter sockerintag, dock märkte 28 % ingen större skillnad hos deras barn.  Resultaten visade att förskolepersonal generellt hade en negativ till neutral attityd till socker i förskolan. Förskolepersonalen tyckte att barnens energi var det som påverkades mest efter intag av socker. Trenden enligt de som arbetar i förskolan är att sockerkonsumtionen i förskolor är nedåtgående och många beskrev sin egen förskola som sockerfri.

    Background: Specialist nurses work on the basis of a person-centered approach, which is

    governed by several laws, including the Health Care Act. Care and care work

    aims to satisfy both individual and basic human needs, regardless of cultural and ethnic

    background. The district nurse's job at a health center includes, among other things, medical reception and telephone counseling. Aim: The purpose of the literature study was to elucidate how district nurses perceive their professional role at a primary health center. Method: A systematic literature study in which Polit & Beck's (2017) nine-step model were used. The result is based on eleven scientific papers that were found using the databases CINAHL, PubMed and SweMed +. Result: Four categories were identified that answered the purpose of the literature study: Insufficient knowledge creates uncertainty, A supportive work environment is important, Stress and lack of time affect work negatively and Creating safety for the patient.  Conclusions: The results shows that the district nursing work is a sought after profession that plays a central role in nursing and coordination at a primary health care center. The experience of the district nurses' professional role is subjective. The result also shows that the district nurses' experience are that they are proud of their professional role.

    Abstract [en]

    Attention-Deficit/Hyperactivity Disorder (ADHD) is one of the most common diagnosis in its area. ADHD is a risk factor for several public health problems such as substance abuse, crime and mental illness and that’s why the diagnosis should be met with early, health promoting, preventing and supportive interventions to both the individual and his/her relatives.The over all aim of the present study is to examine well-being, relationships and substance abuse among adolescent with the diagnosis ADHD with adolescent without ADHD as reference.Data of total 4112 adolescent who, in the school year 2015/2016, were attending 7th grade or 1st grade in high school were obtained d from the Pupil Health Database in the county of Värmland, Sweden. Data included information about diagnosis, well-being, relationships and substance abuse and data were based on the questionnaire, filled out by the adolescent in connection with the mandatory health examinations. Data were analyzed by descriptive statistics and logistic regression and of all the adolescent 269 had the diagnosis ADHD.The adolescent with ADHD generally had lower well-being (aOR = 6,7), poorer relationships (aOR = 4,94) and they used more tobacco (aOR = 3,49), narcotics (aOR = 10,66) and alcohol (aOR = 1,35).The result of the present study confirmed earlier studies done in other countries and showed that adolescent with ADHD is a risk group for several major public health problems. The result shows the importance of early interventions for the target group to promote health and to prevent illness and risky behavior later in life.

    Abstract [en]

    The purpose of this thesis was to find out how teachers can develop the psychological ability of their pupils within the subject of physical education. To find an answer to this, two researchers were interviewed in a qualitative way and four teachers in physical education were interviewed structurally. Current literature was also red. I tried to answer if there are any differences, or similarities, between two scientists’ and four teachers’ in physical education perspectives regarding methods to develop pupils’ psychological ability. The thesis also tries to answer which practical exercises in mental training that could be used in the teaching of physical education and how teachers in physical education can use mental training to motivate the pupils to strive for high grades.

    This study shows that the pupils’ psychological ability can be developed through different mental exercises. To develop the pupils’ self-confidence by giving theme suitable exercises with a degree of difficulty that are on a slightly higher level than what the pupils think they can manage is one way. A similarity between the opinions of the scientists and the teachers in physical education is the importance of good communication with the pupils. Further, the two scientists emphasized the importance of making the pupils involved in the planning and contents of the lessons. The teachers in physical education did not share this opinion.

    To develop the pupils’ self-knowledge and make theme strive for a grade, reasonable for their personal abilities, may increase motivation. In my thesis I have realised that a teacher’s behaviour is also important. The teacher needs to have a sensitive ear, use constructive feedback to the pupils and, to a reasonable extent, get the pupils involved in the planning and the contents of the lessons.

    Introduction: Building a safety net, leadership and containing quality, are some of many responsibilities that comes with the profession nursing. To maintain health care of highest quality knowledge about patient safety is important. Patient safety means prevent medical errors that may cause the patient physical or psychological damage or in worst case scenario, death. Aim: To describe Vietnamese nurses’ conceptions of patient safety. Method: The study had a qualitative design. Data were collected from interviewing nurses at Hué University Hospital with open-ended questions. The collected data has been transcribed and condensed to categories through content analysis to find key sentences which explained Vietnamese nurses’ conception of patient safety. Result: Data analysis regarding Vietnamese nurse´s conception of patient safety resulted in seven categories which affect patient safety in Hue University Hospital, Equipment effecting the patient safety, Knowledge to provide safer care, Procedures used to increase patient safety, Infections in relation to poor patient safety, Nurses´ conception of communication, Documentation effecting patient safety and Inadequate number of nurses. Conclusion: This study shows that lack of good hygiene, insufficient equipment and the great number of patients are the most common factors to affect the patient safety in a negative way in Vietnam. The study shows that the nurses are well aware of what factors affecting the patient safety as well as how to improve patient safety.

    Introduktion: Hemlösa har ett stort tandvårdsbehov och behöver engagerad tandvårdspersonal som är beredda att arbeta kliniskt med denna grupp. Syfte: Syftet är att studera vad som engagerar tandvårdspersonal till att arbeta med hemlösa. Metod: En kvalitativ studie och kvalitativ innehållsanalys har använts. Urval: Studien baseras på intervjuer med en tandläkare och en tandsköterska. Resultat: Resultatet presenteras i två domäner. Domänen Fängslande speglar vad informanterna upplever spännande i kliniskt arbete med hemlösa. Tre kategorier finns under denna, Intresse, Ovillighet bland kollegor och Team-work. Under domänen Medkänsla finns två kategorier Nytta och Välvilja. Där speglas känslor som informanterna upplevde i arbetet med denna grupp. Kategorierna visar vad som engagerar informanterna i deras kliniska arbete med hemlösa. Konklusion: Känslorna som tandvårdspersonalen erhåller vid det kliniska arbetet med hemlösa utgör en grund för det engagemang som personalen känner för arbetet. Dock är det flera faktorer som samverkar till detta engagemang.

    Purpose: Nursing students should be given opportunities to participate in clinical audits during their education. However, audit tools are seldom tested for reliability among nursing students. The aim of this study was to present reliability among nursing students using the instrument PVC assess to assess management of peripheral venous catheters (PVCs) and PVC-related signs of thrombophlebitis. Methods: PVC assess was used to assess 67 inserted PVCs in 60 patients at ten wards at a university hospital. One group of nursing students (n= 4) assessed PVCs at the bedside (inter-rater reliability) and photographs of these PVCs were taken. Another group of students (n= 3) assessed the PVCs in the photographs after 4 weeks (test-retest reliability). To determine reliability, proportion of agreement [P(A)] and Cohen's kappa coefficient (k) were calculated. Results: For bedside assessment of PVCs, P(A) ranged from good to excellent (0.80-1.0) in 55% of the 26 PVC assess items that were tested. P(A) was poor (< 0.70) for two items: "adherence of inner dressing to the skin" and " PVC location." In 81% of the items, k was between moderate and almost perfect: moderate (n= 5), substantial (n= 3), almost perfect (n= 5). For edema at insertion site and two items on PVC dressing, k was fair (0.21-0.40). Regarding test-retest reliability, P(A) varied between good and excellent (0.81-1) in 85%-95% of the items, and the k ranged between moderate and almost perfect (0.41-1) in 90%-95%. Conclusions: PVC assess demonstrated satisfactory reliability among nursing students. However, students need training in how to use the instrument before assessing PVCs.

    Abstract [en]

    Rationale and aims To evaluate the extensive use of peripheral venous catheters (PVCs), including catheter-related complications, a reliable tool for PVC assessment is needed. The aim of this study was to develop such a tool to evaluate PVCs in relation to management, documentation and signs and symptoms of thrombophlebitis (TH), as well as to determine its inter-rater and test-retest reliability. Method The tool development included confirmation of content and face validity. Two groups of registered nurses used the new tool (PVC assess) to assess PVC management and signs of TH independently. Group A (n = 3) assessed 26 items in 67 PVCs bedside (inter-rater reliability). Group B (n = 3) assessed photographs (67 PVCs, 21 items) of the same PVCs as those in Group A with a 4-week interval (test-retest reliability). Proportion of agreement P(A) and Cohen's kappa were calculated to evaluate inter-rater and test-retest reliability. Results Among nurses assessing PVCs at bedside, the P(A) was good to excellent (0.80-1) in 96% of the items in PVC assess. In 80% of the items kappa was substantial to almost perfect (0.61-1). TH sign erythema fell into the fair range (kappa = 0.40). In test-retest reliability analysis the P(A) was within the good and excellent range (0.80-1.0) and kappa varied from moderate to almost perfect (0.41-1.0) in 95% of the items. One item 'outer dressing is clean' was in fair range (0.21-0.40). Conclusions The PVC assess instrument shows satisfactory inter-rater and test-retest reliability. Reliability tests on reviewing documentation remain to be performed.

    Abstract [en]

    Aim. This study aimed at evaluating the outcome of implemented evidence-based clinical guidelines by means of surveying the frequency of thrombophlebitis, nurses’ care, handling and documentation of peripheral intravenous cannulae. Background. Peripheral intravenous cannulae are frequently used for vascular access and, thereby, the patients will be exposed to local and systemic infectious complications. Evidence-based knowledge of how to prevent these complications and how to care for patients with peripheral intravenous cannula is therefore of great importance. Deficient care, handling and documentation of peripheral intravenous cannulae have previously been reported. Design. A cross-sectional survey was conducted by a group of nurses at three wards at a university hospital before and after the implementation of the evidence-based guidelines. Method. A structured observation protocol was used to review the frequency of thrombophlebitis, the nurses’ care, handling and the documentation of peripheral intravenous cannulae in the patient's record. Results. A total of 107 and 99 cannulae respectively were observed before and after the implementation of the guidelines. The frequency of peripheral intravenous cannulae without signs of thrombophlebitis increased by 21% ( P < 0·01) and the use of cannula size 0·8 mm increased by 22% ( P < 0·001). Nurses’ documentation of peripheral intravenous cannula improved significantly ( P < 0·001). Conclusion. We conclude that implementation of the guidelines resulted in significant improvements by means of decreased frequency of signs of thrombophlebitis, increased application of smaller cannula size (0·8 mm), as well as of the nurses’ documentation in the patient's record. Relevance to clinical practice. Further efforts to ameliorate care and handling of peripheral intravenous cannulae are needed. This can be done by means of increasing nurses’ knowledge and recurrent quality reviews. Well-informed patients can also be more involved in the care than is common today.

    Abstract [en]

    Scientific medicine has been successful by ways of an ever more detailed understanding and mastering of bodily functions and dysfunctions. Biomedical research promises new triumphs, but discontent with medical practice is all around. Since several decades this has been acknowledged and discussed. The philosophical traditions of phenomenology and hermeneutics have been proposed as promising ways to approach medical practice, by ways of a richer understanding of the meaning structures of health and illness. In 2000, Swedish philosopher Fredrik Svenaeus published a book where he proposes that the phenomenological hermeneutics of Martin Heidegger and also the reflections on health and illness of Hans-Georg Gadamer offer important ways to approach the nature of medicine. In particular, Svenaeus argues that the goal of medicine is to promote and restore health, and that health ought to be seen as "homelike being-in-the-world". Unhealth, illness, consequently should be understood as a situation where a person's "being-in-the-world" in characterized by that lack of the rhythm, balance and "tune" of everyday living that characterizes not "being at home". In this article, Svenaeus' position is briefly outlined. Questions are raised whether "unhomelikeness" is to be seen as a metaphor, and, if so, if it is a fruitful such. Furthermore, I discuss whether or not a discourse on health and illness in these terms may be misleading in a situation where the ontological presuppositions of Heidegger are lost out of sight and the popular understanding of health psychology predominates. I also approach the question whether Svenaeus' assumptions may inadvertently lead us to an unjustifiably broad understanding of the tasks of medicine. It is finally concluded that Svenaeus phenomenological and hermeneutical approach is both interesting and promising. There are, however, several questions that ought to be pursued further, and the step from philosophical analysis to everyday clinical discourse may be unexpectedly long and risky.

    The nature and scope of medical humanities are under debate. Some regard this field as consisting of those parts of the humanistic sciences that enhance our understanding of clinical practice and of medicine as historical phenomenon. In this article it is argued that aesthetic experience is as crucial to this project as are humanistic studies. To rightly understand what medicine is about we need to acknowledge the equal importance of two modes of understanding, intertwined and mutually reinforcing: the mode of aesthetic imagination and the mode of analytical reflection.

    Abstract [en]

    Is literary experience of any practical relevance to the clinician? This is the overall question addressed by this investigation, which starts by tracing the historical roots of scientific medicine. These are found to be intimately linked to a form of rationality associated with the scientific revolution of the 17th century and with “modernity”. Medical practice, however, is dependent also on another form of rationality associated with what Stephen Toulmin calls “the epistemology of the biographical”. The very core of clinical medicine is shown to be the clinical encounter, an interpretive meeting where the illness experience is at the centre of attention. The physician can reach the goals of medicine only by developing clinical judgement. Clinical judgement is subjected to close analysis and is assumed to be intimately connected to the form of knowledge Aristotle called phronesis.

    In order to explore how literature – drama, novels, poetry – may be related to clinical judgement, a view of literature is presented that emphasizes literature as an invitation to the reader, to be met responsibly and responsively. Literature carries a potential for a widened experience, for a more nuanced perception of reality – and this potential is suggested to be ethically relevant to the practice of medicine. The “narrative rationality” of a literary text constitutes a complement to the rationality pervading scientific medicine.

    The final step in my analysis is a closer exploration of the potential of the literary text to contribute to the growth of clinical judgement, in relation to the challenges of everyday clinical work. Some of the conditions that may facilitate such growth are outlined, but it is also shown that full empirical evidence for the beneficial effects of reading on the clinician reader is beyond reach.

    When a person falls ill, their experience changes - sometimes in a very minor and transient way, sometimes in a decisive and lasting one. ‘Diagnosis’ is often seen as the process of scientifically and objectively identifying the causes of this subjective experience, but is the process and meaning of ‘diagnosis’ really as simple as this implies?

    As this volume of The Medical Humanities Companion argues, diagnoses are an answer to complex human needs that spring from being ill, and are in turn a complex, culturally mediated interaction between individuals, scientific discoveries, social negotiation and historical change. This volume makes visible the complexities and ambiguities involved in giving and receiving diagnoses, and how they shape and are shaped by views on what is real and acceptable, and how people relate to the phenomena of illness.

    Background: Home healthcare today means increasingly advanced healthcare efforts which place high demands on district nurses' knowledge and leadership. The district nurse handles various medical information to nursing staff based on different regulations and guidelines. The district nurse's also have a central leadership role and will work for a safe care. In Sweden, an increase in chronic leg ulcer is seen which affects both the daily life and the quality of life for the victims. An important part of the treatment of leg ulcers is compression therapy. The district nurse's delegates compression therapy to nursing staff in home care. Purpose: Describe district nurses experience of delegating compression therapy in the home health care. Method: Qualitative interview study with inductive approach that was conducted and analyzed based on qualitative content analysis. 12 district nurses from six different municipalities working in municipal home healthcare were interviewed. Result: Results are presented within two generic categories which were, District nurse's approach to delegation and District nurse's experiences the consequences delegation can lead to. These are described with six subcategories each. Conclusion: The study showed that district nurses performed delegation of compression wound care to nursing staff in different ways, personal or general delegation. In general delegation, the activities do not fully comply with laws, regulations and guidelines and the patient's safety can be adversely affected. Their experience is that knowledge, as well as evidence-based work, is the basis for a safe delegation. The patient's safety and quality of life are influenced by the nursing staff's knowledge and interest in performing the delegated task.

    Introduktion: Terminal oro är i denna litteraturstudie en beteckning för en samling av symtom som utgör en särskild form av lidande i livets slutskede, sprunget ur fysisk, emotionell och existentiell utsatthet. Reversibla orsaker endast kan påvisas i uppskattningsvis hälften av fallen, vilket gör terminal oro till ett renodlat omvårdnadsproblem där sjuksköterskans roll är central.

    Syfte: Att belysa sjuksköterskans roll i omvårdnaden av patienter med terminal oro.

    Metod: Polit och Becks (2017) niostegsmodell. Litteratursökningar i Cinahl och PubMed resulterade i elva artiklar som kvalitetsgranskades och analyserades enligt Colaizzis (1978 refererad i Polit & Beck 2017) fenomenologiska metod.

    Resultat: Under databearbetningen framkom fem underkategorier (Att stå utanför, Att känna sig otillräcklig, Att söka samförstånd, Att vårda i relationen, Att skydda patienten), två huvudkategorier (Avstånd, Närhet) samt en övergripande kategori (Strävan efter att trösta).

    Slutsats: Resultatet visar utmaningar i omvårdnaden samt strategier för att hantera dessa. Medvetenhet och kompetens om terminal oro, tydliga rutiner för uppföljning av dess symtom samt reflektion tillsammans med arbetskamrater gav sjuksköterskorna större möjligheter att utföra omvårdnaden på ett tillfredsställande sätt. Närvaro i den dagliga omvårdnaden och ett respektfullt bemötande etablerade en tillitsfull relation med patienterna. När sjuksköterskorna hade god kännedom om patienterna kunde multidimensionella behov mötas och välbefinnande uppnås.

    Abstract [en]

    Bisphenol A (BPA) exposure has been linked to neurodevelopmental disorders and to effects on epigenetic regulation, such as DNA methylation, at genes involved in brain function. High doses of BPA have been shown to change expression and regulation of one such gene, Grin2b, in mice. Yet, if such changes occur at relevant doses in animals and humans has not been addressed. We investigated if low-dose developmental BPA exposure affects DNA methylation and expression of Grin2b in brains of adult rats. Furthermore, we assessed associations between prenatal BPA exposure and Grin2b methylation in 7-year old children. We found that Grin2b mRNA expression was increased and DNA methylation decreased in female, but not in male rats. In humans, prenatal BPA exposure was associated with increased methylation levels in girls. Additionally, Iow APGAR scores, a predictor for increased risk for neurodevelopmental diseases, were associated with higher Grin2b methylation levels in girls. Thus, we could link developmental BPA exposure and Iow APGAR scores to changes in the epigenetic regulation of Grin2b, a gene important for neuronal function, in a sexual dimorphic fashion. Discrepancies in exact locations and directions of the DNA methylation change might reflect differences between species, analysed tissues, exposure level and/or timing.

    Introduktion: Patientsäkerhet har en betydande roll i omvårdnaden och baseras på att skydda patienten från vårdskador. Årligen drabbas upp till 100 000 personer i Sverige av vårdskador, och vårdskador drabbar patienter dagligen. I den moderna omvårdnaden hanterar sköterskorna dagligen stress, som orsakas av stressfaktorer som påverkar sjuksköterskornas utförande av patientsäker vård. Syfte: Litteraturstudiens syfte var att beskriva riskfaktorer som kan påverka patientsäkerheten när sjuksköterskor upplever arbetsrelaterad stress. Metod: Litteraturstudiens grundades på Polit och Becks (2012) niostegsmodell med ett induktivt arbetssätt. Efter urval framkom det 13 artiklar som var relevanta för studien. Efter granskningen av artiklar skapades tre huvudkategorier och en underkategori. Resultat: Litteraturstudiens resultat visas i tre huvudkategorier och en underkategori av riskfaktorer som kan påverka patientsäkerheten negativt. Dessa kategorier var Arbetsbelastning, med underkategorin Tidsbrist, Avbrott i arbetet samt Kommunikation och samarbete. Resultatet visade att många sjuksköterskor upplever höga nivåer av utmattning och utbrändhet, vilket kan påverka patientsäkerheten negativt. Slutsats: När sjuksköterskor upplever arbetsrelaterad stress finns det flera riskfaktorer som kan leda till misstag och vårdskador. Dessa riskfaktorer kan leda till att den arbetsrelaterade stressen ökar, vilket kan leda till ökad utmattning, missnöje samt att sjuksköterskorna slutar på sitt arbete. Arbetsrelaterad stress och riskfaktorer som upplevs av sjuksköterskor på arbetsplatsen ökar även risker för att begå flera misstag och orsakar flera vårdskador som egentligen kunde undvikas.

    Introduction: District nurses meet informal caregivers of different ages, family constellations and in diverse contexts such as health care centers, child health care centre, home care and school health care. In the home, informal caregivers care a large part of the care provider's nursing and healthcare. In addition to family members, there may also be friends who are close to the care recipient. The aim: To describe the district nurse's experiences of caring together with informal caregivers. Method: The method used was a literature study with manifest content analysis. 14 articles were included in the essay analysis. Results: Six categories emerged: building relationships (subcategory: trust and trust), good communication, cooperation (subcategory: involvement of close relatives), support and accessibility, lack of communication (subcategory: conflicts of interests), factors in the family. These categories are summarized in two themes: informal caregivers as resource and informal caregivers as a burden. Conclusion: The results of the study show that the district nurse's experience of caring with informal caregivers is that it can both be a resource and a burden. The district nurses experience is that informal caregivers are a resource if they are included. The results show that the relationship between the nurse and informal caregivers is important for the cooperation and planning of the patient's care in the absence of the district nurse. District nurses need to raise awareness for the encounter with informal caregivers and patients who have other cultural beliefs and lifestyles than their own.

    Abstract [sv]

     Introduktion: Cancer är den andra vanligaste dödsorsaken i Sverige. Cancer drabbar alla åldersgrupper men är vanligare bland äldre. De tre vanligaste behandlingsmetoderna för cancer är strålning, cytostatika och kirurgi. Den vanligaste biverkningen är cancerrelaterad fatigue. Livskvalitet som påverkas definieras som en individs inställning till livet i relation till deras mål, förväntningar, normer och bekymmer. Syfte: Syftet med studien var att belysa patienters upplevelser av cancerrelaterad fatigue och hur den påverkar deras livssituation samt livskvalitet. Metod: Litteraturstudien genomfördes enligt Polit och Becks (2017) nio steg. Tio vetenskapliga artiklar valdes ut efter sökningar gjorda i databaserna CINAHL och PubMed, dessa artiklar presenteras i en artikelmatris (bilaga 1). Artiklarna som användes till studiens resultatdel var fem stycken kvantitativa, fyra stycken kvalitativa och en med mixad metod. Resultat: Utifrån det insamlade materialet utarbetades fem teman som svarade på patienternas upplevelser av cancerrelaterad fatigue och hur livssituationen påverkas samt livskvalitet. Dessa fem teman var värdelöshet/hopplöshet, ensamhet/inte känna sig förstådd, orkeslöshet/kroppslig svaghet, irritation/frustration, och förståelse/acceptans. Slutats: Cancerrelaterad fatigue påverkar patienternas hela tillvaro och hur det upplever olika aspekter i livet. De upplever en förändrad livssituation och att livskvaliteten påverkas negativt. Cancerrelaterad fatigue är ett symtom som påverkar alla patienter på något vis men alla drabbas inte lika hårt. Cancerrelaterad fatigue kan ses som ett holistiskt fenomen med mycket stor komplexitet.

    Abstract [en]

    Introduction : The district nurse makes assessment, refers and provides advice, support and information to the public in telephone consulting. Telephone counseling is a growing part of healthcare and is a complex area of health care, where district nurses encounter many new situations and challenges. District nurses offer self-care to the public in telephone counseling.

    Aim: To identify which factors create opportunities and provide limitations, there is a district nurse's task in telephone counseling in primary care.

    Method: A systematic literature study with descriptive synthesis based on nine scientific articles published 2008-2018. The articles were searched in databases PubMed and Cinahl. The articles were processed with qualitative content analysis.

    Result: Nurses thought it was exhaustive to assess and handle conversations appropriately. Communication skills should be highly developed in the telephone-based meetings. The cognitive fatigue caused the nurse to fail to listen to the caller and led to the nurse asking too few questions. Nurses perceived that computerized decision support was both positive and negative.

    Conclusion: Nurses thought the decision support system was complementary support, it was perceived as a valuable source of information. Communication skills are a very important tool in telephone consulting. Cognitive fatigue due to long workouts had a major impact on the nurses' assessment.
