    Abstract [en]

    AIM:

    The study aimed at describing contributing factors for the progression or regression of pressure ulcers in the care trajectory as they were understood by nurses working in hospitals or community care.

    BACKGROUND:

    The development of pressure ulcers is considered to be connected with early prevention and awareness among nurses and some studies have indicated that the care trajectory may be a weak point.

    DESIGN:

    The study was carried out with a qualitative design.

    METHOD:

    Fifteen nurses from two Swedish hospitals and 15 nurses from community care were interviewed during 2005. Qualitative content analysis was used to make an understanding of patterns possible.

    FINDINGS:

    Three main categories arose, showing that pressure ulcers were considered to be affected in the care trajectory by factors related to the individual patient, to the healthcare personnel and to the healthcare structure. Hospital and community care nurses mostly had corresponding perceptions of these factors.

    CONCLUSION:

    The study both confirmed previous findings and added new knowledge about factors that may affect pressure ulcer in the trajectory of care. The informants' views of nurses' responsibilities and their attitudes towards the care of pressure ulcers could, along with their views on the organisation of care, increase the understanding of the occurrence of pressure ulcers. The need for development and clarification of the organisation and responsibility of pressure ulcer care in the care trajectory was stressed. Relevance to clinical practice. The study highlighted attitudes and values among registered nurses, as well as to how to preserve their commitment and increase their knowledge concerning prevention of pressure ulcers.

    Abstract [en]

    Aim To describe the development and evaluation of a model for a national clinical final examination in the bachelor nursing education. Background  After the transfer of nursing education to the academy, concerns have been raised among nurses, nurse leaders, lecturers and researchers about the nursing students’ clinical competence at the entrance to professional life. Methods  During 2003 to 2005, a collaborative project was carried out between four universities and adjunctive health-care areas supplying clinical placements in Sweden. A two-part examination was agreed upon comprising a written theoretical test and a bedside test. An assessment tool for the bedside test was created. Nursing students, nurses and clinical lecturers participated voluntarily in the evaluation. Results  The model was highly appreciated, and its relevance, usability, and validity were considered quite good for the assessment of nursing students’ clinical competence at the final stage of their education. Several deficiencies were revealed, which led to further development of the model. Conclusions and implications for nursing management  The development and first evaluation of the model proved encouraging for further use, but it needs further evaluation. Involvement of nursing managers is necessary in order to satisfy new demands on competence and staffing of clinical nurses.

    Abstract [sv]

    Ventilatorassocierad pneumoni (VAP) är den vanligaste vårdrelaterande infektionen på intensivvårdsavdelningar (IVA). Orsaken till infektionen är att patienten behöver assisterad andning genom invasiv ventilatorbehandling vilket medför att bakterier får fäste i lungorna och orsakar VAP. Syftet med studien var att genomföra en genomlysning av forskningen om preventiva åtgärder som kan leda till minskad incidens av VAP vid invasiv ventilatorbehandling. Studiens metod var en systematisk litteraturstudie. Tio kvantitativa vetenskapliga artiklar sammanställdes med hjälp Hsieh & Shannons (2005) konventionella kvantitativa innehållsanalys med en induktiv ansats. Resultatet delas in i fyra olika kategorier; Kunskapsnivå, Utbildning, Mobilisera upp och aspirera bort sekret samt Munvård. Resultatet visade att kunskap och utbildning angående preventiva VAP åtgärder ger minskad incidens av VAP. Även åtgärder som god munhygien med framförallt klorhexidin, upprepad munvård samt att avlägsna sekret ger minskad risk att utveckla VAP. Konklusionen är att intensivvårdssjuksköterskan behöver kontinuerlig utbildning för att inte förlora kunskap i preventiva omvårdnadsåtgärder. Kontinuerlig munvård och att mobilisera och avlägsna sekret hos den intuberade patienten är viktiga preventiva åtgärder på en intensivvårdsavdelning för att minska risken för VAP

    Introduktion: Patienter med alkoholmissbruk vårdades inom samtliga delar av hälso- och sjukvården. En av omvårdnadsuppgifterna sjuksköterskan hade vid vård av patienter med alkoholmissbruk var att samtala med patienten om dennes hälsotillstånd. Syfte: Syftet med studien var att belysa sjuksköterskors attityder till patienter med alkoholmissbruk. Metod: Litteraturstudien utfördes med hjälp av Polit och Beck’s (2012) niostegsmodell. Databassökningarna genomfördes i PubMed, CINAHL och PsycINFO, för att artiklar som motsvarade studiens syfte skulle påträffas. Antalet artiklar som svarade till studiens syfte var tio. Resultat: Efter granskning och analys av de tio artiklarna framträdde fyra teman. Dessa var erfarenhet och kompetensens påverkan på attityderna, hotet och våldets påverkan på attityderna, alkoholanvändningens påverkan på attityderna samt sjuksköterskornas attityder till patienterna. Slutsats: Erfarenhet påverkade sjuksköterskornas attityder. Sjuksköterskor som hade utbildning och ökad kunskap kring alkoholmissbruk uppvisade positiva attityder gentemot patienter med alkoholmissbruk. Hot och våld var vanligt förekommande på vissa avdelningar, detta påverkade sjuksköterskornas attityder gentemot patienter med alkoholmissbruk. Det visades att synen på alkohol påverkad sjuksköterskornas inställningar till patienter med alkoholmissbruk. De flesta som vårdade patienter med alkoholmissbruk hade negativa attityder till patienterna. Sjuksköterskors attityder påverkade omvårdnaden för patienter med alkoholmissbruk.

    Abstract [en]

    Aim.  This paper reports on a study of how clients experience living with home mechanical ventilation and how they experience care and supervision of healthcare personnel.

    Background.  The number of people living at home with mechanical ventilators is increasing, and this is considered a successful approach to reducing incapacity and mortality.

    Method.  Qualitative interviews were conducted with 10 service users in 2006. The informants were 18–75 years old and had varying diagnoses and levels of functioning. The interviews were tape recorded, transcribed and analysed by qualitative content analysis.

    Findings.  Two main themes emerged: Theme 1. Having a home ventilator enhances quality of life – a life worth living. The ventilator treatment builds up strength and improves well-being. Participants emphasized that it was important to feel in control of their own situation and had an overriding wish to live a normal and active life; Theme 2. Competence and continuity of healthcare personnel are factors for success. The experience was that competence and follow-up by healthcare personnel varied, and that good quality teaching and information were important.

    Conclusion.  Users of home mechanical ventilators should be active partners in their own care so that their experience is taken into account. It is important for clients having home mechanical ventilation to be empowered and have control in their daily lives, as well as having competent caregivers and continuity of care.

    Abstract [en]

    OBJECTIVES:To investigate registered nurses' perceptions of the patient safety climate in intensive care units and to explore potential predictors for overall perception of safety and frequency of incident reporting. RESEARCH METHODOLOGY/DESIGN: A cross-sectional design was conducted, using the questionnaire Hospital Survey on Patient Safety Culture, measuring 12 patient safety climate dimensions: seven at unit and three at hospital level, two outcomes and in addition two outcome items.

    SETTING:Ten intensive care units (ICUs) in six hospitals in one hospital trust in Norway.

    RESULTS:In total, 220 registered nurses (RNs) responded (72%). Seven of 12 dimensions achieved a RN proportion of positive scores over 55%. Five achieved a lower proportion. Significant differences in RNs' perceptions of patient safety were found between types of units and between the four hospitals. The total variance in the outcome measure explained by the model as a whole was for the outcome dimensions "overall perception of safety" 32%, and "frequency of incident reporting" 32%. The variables at the unit level made a significant contribution to the outcome.

    CONCLUSION:RNs in ICU are most positive to patient safety climate at unit level, hence improvements are needed concerning incident reporting, feedback and communication about errors and organisational learning and continuous improvement.

    Introduktion: KOL är en av de huvudsakliga orsakerna till dödsfall runtom i världen. Tobaksrökning och andra luftföroreningar är de dominerande orsakerna och många yrkesgrupper har en förhöjd risk att drabbas av sjukdomen. Patienter med KOL lider av symtom som dyspné, kronisk hosta och ökad slemproduktion, vilket har en stor påverkan på det dagliga livet. Syfte: Att belysa faktorer som kan påverka upplevelsen av livskvalitet hos patienter med KOL. Metod: Litteraturstudie som baserades på Polit och Becks (2012) niostegsmodell. Databassökningen utfördes i CINAHL samt PubMed. Efter att ha genomgått kvalitetsgranskningar återstod totalt 15 artiklar som svarade på syftet och efter databearbetningen kunde två huvudkategorier urskiljas med tillhörande underkategorier. Resultat: Två huvudkategorier av faktorer kunde utläsas i resultatet med tillhörande mindre underkategorier. Dessa var ”Fysiologiska faktorer” och ”Psykologiska faktorer”. Fysisk aktivitet visade sig vara en betydande faktor för att öka den upplevda livskvaliteten samtidigt som besvär med ångest och depression minskade den upplevda livskvaliteten. Slutsats: Litteraturstudien visade på en mängd olika faktorer som kan ha en inverkan på den upplevda livskvaliteten. Sjuksköterskor bör därför ha kunskap om dessa för att på ett effektivt sätt kunna hjälpa och vägleda sina patienter till att möjliggöra en förhöjd upplevelse av livskvalitet.

    Pain is the most common symptom following laparoscopic cholecystectomy (LC) and might delay discharge from hospital after day surgery. A patient’s ability to manage stressful situations can be assessed by the sense of coherence (SOC) and has been proposed to predict health. The aim of this study was to investigate predictors of average pain the first postoperative week after LC, and predictors of changes in perceived health, with special reference to individual coping resources measured by the Sense of Coherence Scale. Furthermore, a test–re-test was performed on SOC to evaluate the stability in the context of LC surgery.  Method:  Seventy-three patients completed questionnaires about SOC, health status, pain, anxiety, symptom occurrence and symptom distress preoperatively, postoperative day 1–7 and after 1 and 6 months following LC. Results: By multiple regression, 23% of the variability in pain intensity could be explained by the variables age, SOC and education. Age was the strongest predictor. Further, 19% of the change in health between day 7 and 1 month could be explained by the two variables symptom distress the first postoperative day and SOC. The test–re-test of SOC had a correlation coefficient (r) of 0.55. Forty-six patients (63%) remained within ± 10% of their preoperative SOC score at 6 months, 11 patients (15%) decreased and 16 patients (22%) increased their SOC values.  Conclusion: SOC was found to be a significant but weak predictor of pain intensity the first week after LC. Furthermore, patients scoring low SOC values experienced a delay in their health improvement. SOC was more unstable over time than previously suggested. Further, interventional studies are needed to clarify if SOC might be a clinically useful measure to identify vulnerable patients undergoing LC surgery.

    Abstract [sv]

    Litteraturstudiens syfte var att belysa andliga behov hos patienter i palliativt skede och sjuksköterskans roll kring behoven. En niostegsmodell inspirerad av Polit och Beck användes som metod för databearbetning. Databaserna CINAHL, PubMed och PsycInfo användes till litteratursökningen. Artiklar som framkom genomgick tre urval där de bland annat kvalitetsgranskades. Efter urvalen återstod 13 artiklar som låg till grund för reslutatet i litteraturstudien. Artiklarnas resultat sammanställdes med relevanta data och kategorierades utifrån syftet. Resultatet blev tvådelat där fem kategorier beskrev andliga behov: Finna styrka i tron på högre makter, Stimulans ger själsligt välmående, Få ett avslut, Interaktioner med andra och Hitta förhållnings-sätt till livet, döden och sjukdomen. Kategorierna för omvårdnadsinterventioner blev tre: Uppmärksamma behov, Vara medmänniska och Förmedla kontakt. Slutsatsen var att andliga behov är subjektiva och påverkas av många faktorer. Omvårdnadsinterventioner innefattar notering av behov och konkreta åtgärder.

    Introduktion: Sjuksköterskan möter dagligen närstående i sin yrkesroll inom nästan alla vårdkontext och det ingår i sjuksköterskans profession att involvera patientens närstående i vården. Kvaliteten av mötet med närstående påverkas av sjuksköterskans attityd angående vikten av att involvera de närstående i vården. Syfte: Syftet var att belysa sjuksköterskans attityd till att involvera patientens närstående i vården. Metod: En litteraturstudie enligt Polit och Becks (2017) niostegsmodell. Efter litteratursökningar i CINAHL, PubMed och PsycINFO identifierades elva artiklar som svarade på studiens syfte. Samtliga artiklar kvalitetsgranskades och relevant information analyserades, bearbetades och sammanställdes. Resultat: Litteraturstudien resulterade i tre huvudkategorier: attityder om närstående som betydelsefulla, attityder om närstående som besvärande samt faktorer som kan påverka attityden till närstående, med totalt tio tillhörande underkategorier. Slutsats: Patientens närstående anses vara betydelsefulla och en förutsättning för god vård. Närstående kan också upplevas som tidskrävande och att de hindrar sjuksköterskan i arbetet. Utbildning och kunskap angående bemötande av patientens närstående är viktigt och bidrar till att sjuksköterskan får en mer positiv attityd till de närstående. Närstående upplevs besitta flera betydelsefulla resurser och genom att ta tillvara på dem kan vården förbättras.
