    Abstract [en]

    In this article we explore children’s experiences of a Sport for All Programme (SAP) in school. The purpose of the ‘Sport for All’ policy is to give children an opportunity to try different sports, with a focus on play and motor learning rather than organized competition. In this study, we approach children as active agents in constructing the SAP practice in line with the theoretical standpoint that children are competent social actors with agency. The study is based on repeated focus group interviews with 15 ten-year-old children (9 girls, 6 boys) participating in a SAP in the western part of Sweden. The children perceived the SAP as sessions with less emphasis on performance and without organized competitions; it was described as a ‘low threshold’ activity. However, the result showed that the SAP activity was not free from performance-oriented principles. The Sport for All pedagogy needs to consider and handle the consequences of competition principles, especially if the participating children and leaders have a performance ethos.

    Introduction

    The aim of the study was to investigate children’s experiences of The Sport for All programme in school (SAP) (idrottsskolan). The purpose of the SAP is to give children an opportunity to try different sports, with a focus on play and motor learning rather than organized competition. Methods

    The study is based on repeated focus group interviews with 15 ten-year-old children (9 girls; 6 boys) participating in a SAP in the western part of Sweden. In this study we approach children as active agents in constructing the SAP practice in line with the theoretical standpoint that children are competent social actors with agency (Alanen, 2014; James & James, 2004).

    Results

    The children perceived the SAP as sessions with less emphasis on performance and without organized competitions; it was described as a “low threshold” activity. However, the children expressed that sometimes was a “competitive atmosphere” present in the practice. According to the children, this atmosphere was to a large extent created by the behaviour of participating children with a strong winning attitude. This resulted in that children who do not want to participate or could act in a more pronounced "performance-based atmosphere" perceived themselves as excluded from the activities.

    Conclusions

    The result shows how the logic in the SAP is changeable and that this is developed through children’s agency and some children have more power to influence the activities than others. The Sport for All pedagogy needs to consider and handle the consequences of competition principles, especially if the participating children and leaders have a performance ethos.

    ”Vi kallade oss alltid SMARTISARNA”: En uppföljningsstudie av ett hälsofrämjande skolprojekt2018Rapport (Övrigt vetenskapligt)

    De flesta barn och ungdomar i Sverige är inte tillräckligt fysiskt aktiva. I Sverige idag är det få barn som når den rekommenderade mängden fysisk aktivitet på 60 minuter per dag. Forskningsgenomgångar har dokumenterat att skolbaserade interventioner har möjlighet att förbättra den fysiska statusen hos elever. Värmlands Idrottsförbund och Friskvården i Värmland beslutade 2004 att göra en satsning på ett hälsofrämjande skolprojekt – som fick namnet SMARTIS. Huvudmålet med SMARTIS var att öka barnens fysiska aktivitetsgrad i skolan och på fritiden samt att förbättra den fysiska förmågan. I föreliggande rapport studeras elevers beskrivningar av SMARTIS betydelse för deras levnadsvanor under och efter projektet. Studiens design omfattade individuella intervjuer och en gruppintervju med elever som tidigare deltagit i SMARTIS. Resultatet analyseras och diskuteras utifrån hur eleverna upplevde SMARTIS under sin tid på skolan och vilken betydelse SMARTIS har haft för det liv de lever idag. I rapporten synliggörs att för att ge alla elever möjlighet att utveckla varaktiga beteenden och vanor utanför skolan bör hälsofrämjande arbete i skolan vara relevant för eleverna och att de kan relatera de aktiviteter som genomförs till sig själva och deras vardagliga liv.

    Abstract [en]

    The purpose of this article is to study children and engaged adults’ experiences of participating in a Sport for All Programme in school (SAP). Fifteen individual interviews were conducted with one programme manager, three schoolteachers, two sport club leaders, and nine children (five girls and four boys aged 9-10). Bourdieu’s (1990) as well as Lave and Wenger’s (1991) theoretical models were used as the analytic tools to study the SAP. The analysis reveals that the organization and the practice of the activities are noticeably related to conventionally organized sport. The analysis also shows that the SAP is a new way of organizing and practicing children’s sport – with its own logic. The absence of organized competition, the less intensive activities, the greater flexibility and variation in the SAP appear to challenge the conscious and unconscious idea of the meaning and function of competitive sport, especially in children’s sport. Thus, the SAP concept calls for a new way of approaching children’s sport, and new demands on those who organize and lead the activities.

    Abstract [en]

    Introduction

    The aim of the study was to investigate children’s experiences of The Sport for All programme in school (SAP) (idrottsskolan). The specific purpose with this paper was to investigate if boys and girls have equality accessibility to participate in the SAP. The purpose of the SAP is to give children an opportunity to try different sports, with a focus on play and motor learning rather than organized competition.

    Methods

    This article is based on repeated focus group interviews with one group of 7 ten-year-old girls participating in a SAP in the western part of Sweden. The girls had no, or limited experience, of practice organized sport on their leisure time. The girl’s lived in a low socioeconomic district with a high proportion of immigrants. In the district there is few sports clubs.

    Results

    The result shows how the boys have the power to dominate the practice and influence the girls’ experiences of the SAP. And sometimes the boys take over and control the practice in so considerable proportions that the girls find it difficult to participate in the SAP. The study shows that the boys have a better opportunity to use their agency (Alanen, 2014), partly in cooperation with the leaders, and be a part of and influence the sport activities in the SAP.

     Conclusions

    The girls in the study felt that they were disparaged and excluded cause of the boys’ behaviour. Hence, it is absolutely necessarily that organizer of the SAP pay attention to gender issues and work out guidelines and strategies so that both girls and boys feel included in the SAP:s all activities. As has been discussed before (Flintoff, 2008), and according the results in this study, it requires more than to just start up sport activities and hope that more alternatives will boost more and new participants.

    Introduktion

    Detta abstract bygger på preliminära resultat från ett forskningsprojekt med syfte att studera hur idrottsrörelsens arbete med inkludering av nyanlända ungdomar realiseras i idrottsföreningars verksamhet. Mot bakgrund av den stora migrationen de senaste åren har idrotten setts som en viktig arena för att etablera individer med migrationsbakgrund i samhället. Synen på idrottens betydelse är inte minst tydligt då regeringen ger Riksidrottsförbundet (RF) 64 miljoner kronor per år under åren 2015-2018 för att arbeta med nyanlända. RF skriver själva: ”Regeringen ser den kraft som idrotten är och har anslagit medel för idrottsrörelsens etablering av nyanlända.” (RF, 2017).

    Syfte & teoretisk ram

    Syftet med den del av studien som presenteras i föreliggande abstract är att undersöka idrottledares delade upplevelser av att arbeta med inkludering för att få en ökad förståelse för under vilka villkor nyanlända har möjlighet att idrotta. I projektet utgör barndomssociologi (James & James, 2004) och Sayads migrationssociologi (Sayad & Bourdieu, 2004) teoretisk referensram.

    Metod

    I projektet används en fallstudiedesign (Yin, 2007; Merriam, 1994), där distriktsförbundet Värmlandsidrotten utgör ett fall. Resultatet som redovisas här bygger på fyra fokusgruppsintervjuer med totalt 21 idrottsledare (6 kvinnor och 15 män). Majoriteten av ledarna (19 stycken) hade inte migrationsbakgrund. Ledarna representerar en stor variation av idrotter och former av föreningar från olika geografiska platser i Värmland. Ledarna valdes ut mot bakgrund av att de har arbetat med inkludering av nyanlända i sin förening och att de har fått extra ekonomiskt stöd från Värmlandsidrotten för sitt arbete.

    Resultat

    Ett genomgående tema som framträdde i resultatet var att nyanländas idrottskompetens upplevdes som det främsta hindret för deras möjligheter att idrotta i föreningarna.  Idrottsledarna diskuterade att de nyanlända är på en annan ”nivå”, en lägre nivå, som gör det svårt att inkludera dem i föreningens ordinarie verksamhet. Det handlade då inte enbart om att de inte har deltagit i den specifika idrotten tidigare utan också om att de inte har utövat idrotten ”på riktigt”, på det sätt som idrotten utövas och organiseras i Sverige. Det nämndes bland annat att de mest har ”lekt” idrott och inte har kunskap om exempelvis regler och taktik. Därför var mycket av den verksamhet som föreningarna organiserade olika former av temporära ”prova på” verksamheter som skulle fungera som en sluss in i den ordinarie idrottsverksamheten. Analysen visade att problematiken att inkludera nyanlända i den ordinarie verksamheten hade bidragit till att utmana ledarnas föreställningar om vad svensk idrott är och vilka hinder verksamheten har för nyanlända och även andra grupper av unga med begränsad erfarenhet av traditionell föreningsidrott.  Vidare bidrog det till en ökad diskussion i föreningarna om vad ”ordinarie” verksamhet är och vad för strukturer ”vi vill ha”. Eller som en ledare sa i intervjuerna ”ska vi knuffa dem över trösklarna eller ska vi sänka trösklarna”.

    Diskussion och slutsatser

    Resultatet indikerar att migrationen till Sverige kan påverka ledare och föreningar att reflektera och omvärdera förutsättningarna för tillgängligheten till sin egen verksamhet och då inte enbart för nyanlända utan för barn och unga generellt. Föreningarnas arbete med nyanlända har uppmärksammat ett behov av en ny organisering av deras idrott för att sänka trösklarna och öka tillgängligheten till föreningens verksamhet. Det gäller dock att ta hänsyn till att föreningarna i studien aktivt har sökt medel för sitt arbete och också fått medel beviljade i dialog med Värmlandsidrotten. Ledarna i studien kan därmed ha en högre medevetenhet än andra ledare från andra föreningar om idrottsrörelsens arbete Strategi 2025 (RF, 2017) om att sänka synliga och osynliga trösklar.

    Abstract [en]

    The purpose of this study is to examine dropout reasons and various types ofdropouts in relation to demographic variables, various types of sports, physicalactivity, and underlying motivational processes. Retrospective data was collected fromthree cohorts including 1,176 participants, of which 712 stated that they hadsometimes dropped out of organized sport. Findings showed that time-related reasonswere the primary causes for dropping out. It was also revealed that athletes with lowversus high levels of perceived physical competence dropped out to a higher extentbecause of experiencing too much pressure, and athletes reporting low versus highsocial competence withdrew to a higher extent because they did not like theirteammates. Results also showed that girls with a foreign background had a higher rateof dropouts compared to boys with the same background. Girls with foreignbackgrounds were also found to be less physically active after dropping out fromorganized sports.

    Introduktion: Överrapportering är en viktig del av sjuksköterskans arbetsuppgifter och kan utföras med hjälp av olika metoder. Bedside är en rapporteringsmetod som kan användas i vården. Syfte: Att belysa sjuksköterskors erfarenheter av att rapportera bedside på somatiska vårdavdelningar. Metod: En litteraturstudie baserad på Polit & Becks (2016) nio steg. Artiklar söktes med relevanta sökord i databaserna PubMed och CINAHL, därefter gjordes tre urval som resulterade i 13 artiklar med både kvalitativ och kvantitativ design. Databearbetningen utfördes systematiskt och koder organiserades efter inriktning. Resultat: Fyra kategorier om sjuksköteskans erfarenheter  identifierades Ett utökat informationsutbyte, Ett professionellt samarbete, Att involvera patienten och Att skydda integriteten. Kategorierna belyste både för- och nackdelar med bedsiderapportering från sjuksköterskors erfarenheter av metoden. Slutsats: Bedsiderapportering upplevdes förbättra vårdkvaliteten men var problematisk ur sekretessynpunkt.

    Introduktion: Smärta är ett av de vanligaste hälsoproblemen. Sjuksköterskan har en viktig roll vid bedömning av patientens smärta. Det behövs en bred och hög kompetens inom området smärta och hur den ska bedömas samt behandlas för att patienten ska få rätt vård. Syfte: Syftet var att undersöka vad som påverkar sjuksköterskans förhållningssätt vid bedömning av smärta. Metod: En litteraturstudie utifrån Polit och Beck’s (2016) nio steg genomfördes. Litteratursökningar gjordes i databaserna PubMed och CINAHL. Kvalitetsgranskningen utfördes med Polit och Beck’s (2016) kvalitativa och kvantitativa granskningsmallar. Detta ledde till att tio artiklar analyserades. Resultat: Resultatet består av följande tre teman: Varierande kunskap, betydelse av sjuksköterskans ålder, kön och kompetens och att tro på patienten. Slutsats: Resultatet visade på sjuksköterskans bristande förhållningssätt gällande kunskap och attityd vid bedömning och dokumentation av smärta. Utbildning ökar kunskapen hos sjuksköterskan.     Aims and objectives To present results from interviews of older people living in nursing homes, on how they experience freedom. Background We know that freedom is an existential human matter, and research shows that freedom remains important throughout life. Freedom is also important for older people, but further research is needed to determine how these people experience their freedom. The background for this article was a Scandinavian study that occurred in nursing homes; the purpose of the study was to gain knowledge about whether the residents felt that their dignity was maintained and respected. Design The design was hermeneutic, with qualitative research interviews. MethodTwenty-eight residents living in nursing homes in Denmark, Sweden and Norway were interviewed. Collecting tools used were an interview guide and also a tape recorder. Researchers in the three countries performed the interviews. The data were transcribed and analysed on three levels of hermeneutic interpretation. Results To have their freedom was emphasised as very important according to their experience of having their dignity taken care of. The following main themes emerged: (a) Autonomy or paternalism; (b) Inner and outer freedom; and (c) Dependence as an extra burden. ConclusionsResidents in a nursing home may experience the feeling of having lost their freedom. This conclusion has implications for healthcare professionals and researchers, as it is important for residents in nursing homes to feel that they still have their freedom. Relevance to clinical practiceIn clinical practice, it is important and valuable for the staff to consider how they can help older people feel that they still have their freedom.

    Aims and objectives: The aim of this cross-country Scandinavian study was to explore how residents in nursing homes experience that their dignity is promoted and attended to. This is one part of the Scandinavian project in which we interviewed residents, relatives and staff members.

    Background: The main subject concerns the dignity of residents of nursing homes for older people. This article brings forward results from interviews of relatives on how they experience that the dignity is met, promoted and attended to for their next of kin.

    Design: The study was qualitative with a hermeneutic approach.

    Methods: Qualitative research interviews of 28 relatives of residents at six participating nursing homes in Scandinavia. The results derive from analysis of the interviews using Kvale's three levels of interpretation; self-understanding, common sense and a theoretical understanding.

    Results: The following themes emerged, from the perspective of the relatives, concerning what was deemed important to the resident according his existential needs and concerns: a). To have a comfortable, homely and practical room. b). To have close contact with family, friends and with the staff. c).To have aesthetic needs and concerns attended to. d). To have ethical needs and intrinsic values attended to. e). To have cultural and spiritual needs and concerns attended to.

    Conclusion: The results provide more substance to the theme and are all important in terms of the residents' feeling of worthiness and dignity. In general it seemed that the relatives experienced a positive encounter with the staff, but it was also mentioned that staff members were not confronted about episodes that were undignified and disgraceful. This could be a sign or expression that they were worried that negative responses or complaints could lead to a kind of reprisal against the resident and to indignity.

    The aim of the study was to investigate whether interprofessional education (IPE) and interprofessional collaboration (IPC) during the educational program had an impact on prehospital emergency care nurses' (PECN) self-reported competence towards the end of the study program. A cross-sectional study using the Nurse Professional Competence (NPC) Scale was conducted. A comparison was made between PECN students from Finland who experienced IPE and IPC in the clinical setting, and PECN students from Sweden with no IPE and a low level of IPC. Forty-one students participated (Finnish n = 19, Swedish n = 22). The self-reported competence was higher among the Swedish students. A statistically significant difference was found in one competence area; legislation in nursing and safety planning (p < 0.01). The Finnish students scored significantly higher on items related to interprofessional teamwork. Both the Swedish and Finnish students' self-reported professional competence was relatively low according to the NPC Scale. Increasing IPC and IPE in combination with offering a higher academic degree may be an option when developing the ambulance service and the study program for PECNs. (C) 2017 Elsevier Ltd.     Introduktion: Under de senaste decennierna har organdonation genomgått en snabb utveckling. Tack vare utvecklade operationsmetoder samt ny avancerad teknik har fler människoliv kunnat räddas.I sin profession strävar operationssjuksköterskor för att bevara patientens autonomi, integritet och värdighet. Vid en organdonationsoperation strävar operationssjuksköterskor även för att bevara vitaliteten i de organ som skadoneras. Syfte: Att beskriva operationssjuksköterskors upplevelser i samband med organdonationsoperationer med hjärndöd donator. Metod:Litteraturstudie som följer Polit & Becks (2012) niostegsmodell.Databaserna CINAHL och PubMed användes i litteratursökningen. Totalt resulterade sökningarna i tio artiklar som sedan granskades. Nio artiklar inkluderades i studiens resultat. Åtta av artiklarna var kvalitativa och en var kvantitativ artikel. Resultat: Ett tema ”Ett liv slocknar medan ett annat räddas”och fyra kategorier utifrån operationssjuksköterskors upplevelser identifierades: emotionella upplevelser,copingstrategier för att klara av organdonationsoperationer, samarbetet med transplantationsteamet och övrig operationspersonal samt upplevelser om donatorn och dennes närstående. Konklusion: Genom att agera professionellt och fokusera på själva operationsförfarandet kunde operationssjuksköterskor stöta bort sina egna känslor och genomföra organdonationsoperationen. Trots att en organdonationsoperation upplevdes av operationssjuksköterskor som en stressig och utmanande arbetsuppgift, satte de sina egna känslor åt sidan och samarbetade med de övriga i operationsteamet. De handlade med respekt och empati gentemot den hjärndöde donatorn.

    This article is a report of the fourth meeting of the Harmonising Outcome Measures for Eczema (HOME) initiative held in Malmo, Sweden on 23-24 April 2015 (HOME IV). The aim of the meeting was to achieve consensus over the preferred outcome instruments for measuring patient-reported symptoms and quality of life for the HOME core outcome set for atopic eczema (AE). Following presentations, which included data from systematic reviews, consensus discussions were held in a mixture of whole group and small group discussions. Small groups were allocated a priori to ensure representation of different stakeholders and countries. Decisions were voted on using electronic keypads. For the patient-reported symptoms, the group agreed by vote that itch, sleep loss, dryness, redness/inflamed skin and irritated skin were all considered essential aspects of AE symptoms. Many instruments for capturing patient-reported symptoms were discussed [ including the Patient-Oriented SCOring Atopic Dermatitis index, Patient-Oriented Eczema Measure (POEM), Self-Administered Eczema Area and Severity Index, Itch Severity Scale, Atopic Dermatitis Quickscore and the Nottingham Eczema Severity Score] and, by consensus, POEM was selected as the preferred instrument to measure patient-reported symptoms. Further work is needed to determine the reliability and measurement error of POEM. Further work is also required to establish the importance of pain/soreness and the importance of collecting information regarding the intensity of symptoms in addition to their frequency. Much of the discussion on quality of life concerned the Dermatology Life Quality Index and Quality of Life Index for Atopic Dermatitis; however, consensus on a preferred instrument for measuring this domain could not be reached. In summary, POEM is recommended as the HOME core outcome instrument for measuring AE symptoms.

    This is the report from the fifth meeting of the Harmonising Outcome Measures for Eczema initiative (HOME V). The meeting was held on 12-14 June 2017 in Nantes, France, with 81 participants. The main aims of the meeting were (i) to achieve consensus over the definition of the core domain of long-term control and how to measure it and (ii) to prioritize future areas of research for the measurement of the core domain of quality of life (QoL) in children. Moderated whole-group and small-group consensus discussions were informed by presentations of qualitative studies, systematic reviews and validation studies. Small-group allocations were performed a priori to ensure that each group included different stakeholders from a variety of geographical regions. Anonymous whole-group voting was carried out using handheld electronic voting pads according to pre-defined consensus rules. It was agreed by consensus that the long-term control domain should include signs, symptoms, quality of life and a patient global instrument. The group agreed that itch intensity should be measured when assessing long-term control of eczema in addition to the frequency of itch captured by the symptoms domain. There was no recommendation of an instrument for the core outcome domain of quality of life in children, but existing instruments were assessed for face validity and feasibility, and future work that will facilitate the recommendation of an instrument was agreed upon.

    BACKGROUND: Advances in research and technology coupled with an increased cancer incidence and prevalence have resulted in significant expansion of cancer nurse role, in order to meet the growing demands and expectations of people affected by cancer (PABC). Cancer nurses are also tasked with delivering an increasing number of complex interventions as a result of ongoing clinical trials in cancer research. However much of this innovation is undocumented, and we have little insight about the nature of novel interventions currently being designed or delivered by cancer nurses.

    OBJECTIVES: To identify and synthesise the available evidence from clinical trials on interventions delivered or facilitated by cancer nurses.

    DATA SOURCES AND REVIEW METHODS: A systematic review of randomised controlled trials (RCT), quasi-RCTs and controlled before and after studies (CBA) of cancer nursing interventions aimed at improving the experience and outcomes of PABC. Ten electronic databases (CENTRAL, MEDLINE, AMED, CINAHL, EMBASE, Epistemonikos, CDSR, DARE, HTA, WHO ICTRP) were searched between 01 January 2000 and 31 May 2016. No language restrictions were applied. Bibliographies of selected studies and relevant Cochrane reviews were also hand-searched. Interventions delivered by cancer nurses were classified according to the OMAHA System. Heat maps were used to highlight the volume of evidence available for different cancer groups, intervention types and stage of cancer care continuum.

    RESULTS: The search identified 22,450 records; we screened 16,169 abstracts and considered 925 full papers, of which 214 studies (247,550 participants) were included in the evidence synthesis. The majority of studies were conducted in Europe (n = 79) and USA (n = 74). Interventions were delivered across the cancer continuum from prevention and risk reduction to survivorship, with the majority of interventions delivered during the treatment phase (n = 137). Most studies (131/214) had a teaching, guidance or counselling component. Cancer nurse interventions were targeted at primarily breast, prostate or multiple cancers. No studies were conducted in brain, sarcoma or other rare cancer types. The majority of the studies (n = 153) were nurse-led and delivered by specialist cancer nurses (n = 74) or advanced cancer nurses (n = 29), although the quality of reporting was poor.

    CONCLUSIONS: To the best of our knowledge, this is the first review to synthesise evidence from intervention studies across the entire cancer spectrum. As such, this work provides new insights into the nature of the contribution that cancer nurses have made to evidence-based innovations, as well as highlighting areas in which cancer nursing trials can be developed in the future.

    Abstract [en]

    Qualitative reporting of home indoor moisture problems predicts respiratory diseases. However, causal agents underlying such qualitative markers remain unknown. In the homes of 198 multiple allergic case children and 202 controls in Sweden, we cultivated culturable fungi by directly plating dust, and quantified (1-3, 1-6)-beta-d-glucan and ergosterol in dust samples from the child's bedroom. We examined the relationship between these fungal agents and degree of parent or inspector-reported home indoor dampness, and microbiological laboratory's mold index. We also compared the concentrations of these agents between multiple allergic cases and healthy controls, as well as IgE-sensitization among cases. The concentrations of culturable fungal agents were comparable between houses with parent and inspector-reported mold issues and those without. There were no differences in concentrations of the individual or the total summed culturable fungi, (1-3, 1-6)-beta-d-glucan, and ergosterol between the controls and the multiple allergic case children, or individual diagnosis of asthma, rhinitis, or eczema. Culturable fungi, (1-3, 1-6)-beta-d-glucan, and ergosterol in dust were not associated with qualitative markers of indoor dampness or mold or indoor humidity. Furthermore, these agents in dust samples were not associated with any health outcomes in the children.

    Abstract [en]

    Background: The question regarding the true sources of the purported microbial volatile organic compounds (MVOCs) remains unanswered. Objective: To identify microbial, as well as non-microbial sources of 28 compounds, which are commonly accepted as microbial VOCs (i.e. primary outcome of interest is Sigma 28 VOCs). Methods: In a cross-sectional investigation of 390 homes, six building inspectors assessed water/mold damage, took air and dust samples, and measured environmental conditions (i.e., absolute humidity (AH, g/m(3)), temperature (degrees C), ventilation rate (ACH)). The air sample was analyzed for volatile organic compounds (mu g/m(3)) and; dust samples were analyzed for total viable fungal concentration (CFU/g) and six phthalates (mg/g dust). Four benchmark variables of the underlying sources were defined as highest quartile categories of: 1) the total concentration of 17 propylene glycol and propylene glycol ethers (Sigma 17 PGEs) in the air sample; 2) 2,2,4-trimethyl-1,3-pentanediol monoisobutyrate (TMPD-MIB) in the air sample; 3) semi-quantitative mold index; and 4) total fungal load (CFU/g). Results: Within severely damp homes, co-occurrence of the highest quartile concentration of either E17 PGEs or TMPD-MIB were respectively associated with a significantly higher median concentration of Sigma 28 VOCs (8.05 and 1338 mu g/m(3), respectively) compared to the reference homes (430 and 4.86 mu g/m(3), respectively, both Ps <= 0.002). Furthermore, the homes within the highest quartile range for Sigma fungal load as well as AH were associated with a significantly increased median Sigma 28 VOCs compared to the reference group (8.74 vs. 4.32 mu g/m(3), P=0.001). Within the final model of multiple indoor sources on E 28 VOCs, one natural log-unit increase in summed concentration of Sigma 17 PGEs, plus TMPD-MIB (Sigma 17 PGEs TMPD-MIB) was associated with 1.8-times (95% CI, 1.3-2.5), greater likelihood of having a highest quartile of Sigma 28 VOCs, after adjusting for absolute humidity, history of repainting at least one room, ventilation rate, and mold index (P-value =0.001). Homes deemed severely mold damaged (i.e., mold index =1) were associated with 1.7-times (95% CI, 0.8-3.6), greater likelihood of having a highest quartile of Sigma 28 VOCs, even though such likelihood was not significant (P-value =0.164). In addition, absolute humidity appeared to positively interact with mold index to significantly elevate the prevalence of the highest quartile category of Sigma 28 VOCs. Conclusion: The indoor concentration of Sigma 28 VOCs, which are widely accepted as MVOCs, are significantly associated with the markers of synthetic (i.e. E17 PGEs and TMPD-MIB), and to less extent, microbial (i.e., mold index) sources.     Background: Risk of indoor exposure to volatile organic compounds of purported microbial origin on childhood symptoms of wheezing, rhinitis, and/or eczema, and doctor-diagnosed asthma, rhinitis, and eczema, respectively, remain unclear. Objective: To test hypotheses that total sum of 28 microbial volatile organic compounds (Sigma 26 MVOCs): 1) poses independent risk on doctor-diagnosed asthma, rhinitis, and eczema, respectively, as well as multiple symptom presentation with a minimum of the two of the above conditions (i.e. case); 2) is associated with significant interaction with absolute humidity (AH) on additive scale. Methods: In a case-control investigation, 198 cases and 202 controls were examined during November 2001 March 2002 period through homeindoor air sampling, air quality inspection, and health outcome ascertainment. Results: Not only the Sigma 28 MVOCs but also the global MVOC index were significantly higher within the homes of the cases with a high AH, compared to the controls with a low AH (all Ps < 0.001). Only the cases, but not the controls, were associated with a dose-dependent increase in the exposure variables of interest (Sigma 28 MVOCs) per quartile increase in AH (P < 0.0001 for the cases; P = 0.780 for the controls). Only among the children who live in a high AH homes, a natural log (ln)-unit of Sigma 28 MVOCs was associated with 2.5-times greater odds of the case status (95% CI, 1.0-6.2; P = 0.046), compared to 0.7-times the odds (95% CI, 0.4-1.0; P = 0.074) of the same outcome among the low AH homes. Specifically, joint exposure to a high MVOCs and high AH was associated with 2.6-times greater odds of the doctor-diagnosed asthma status (95% CI, 0.7-8.91; P = 0.137). Conclusion: Joint occurrence of high Sigma 28 MVOCs and AH was associatedwith a significant increase in the case status and asthma risks in an additive scale.

    A cross-sectional study on the home environment and asthma and allergy in children was carried out among children 1-8 years old in Seoul, South Korea from 2009 to 2010. Questionnaires were distributed to 5107 parents through daycare centers and kindergartens; 2755 parents responded, a response rate of 54%. Seven percent and 23% of children were reported to have doctor-diagnosed asthma and hay fever, respectively. A majority (57%) of the families reported having PVC flooring in child's or parents' bedroom. More than 96% of homes used a floor heating system. PVC was used more often as a floor covering in single family houses than in apartments (67% vs. 49%, p < 0.001). PVC flooring was significantly associated with eczema in the previous 6 months (AOR 1.54,95% Cl 1.13-2.09) when adjusted for gender, age, family allergy, socioeconomic status and environmental tobacco smoke. Older buildings tended to have dampness problems, and, consequently, were positively correlated with the prevalence of wheeze. Floor moisture significantly increased the association between PVC and symptoms of wheezing (AOR 2.57, 95% Cl 1.36-4.82) and eczema (AOR 1.97, 95% Cl 1.18-3.28). Apartments without mechanical ventilation in bedrooms were associated with a slight increase in asthma and allergy among children. This study suggests that building characteristics and home exposure can partly explain recent increases in asthma and allergy among children in Seoul. (c) 2014 Elsevier Ltd. All rights reserved.

    I dagsläget finns effektiv behandling för cervixcancer med en god överlevnadsstatistik i Sverige och delar av västvärlden. All typ av behandling för cervixcancer kan medföra olika konsekvenser som kan leda till sexuell dysfunktion och en påverkad kroppsbild. SYFTE: Att identifiera vilka faktorer som bidrar till sexuell dysfunktion och en negativt påverkad kroppsbild hos kvinnor som behandlats för cervixcancer. METOD: Polit och Beck (2012) niostegsmodell följdes för att systematiskt utföra denna litteraturstudie. CINAHL och PubMed användes som databaser för att hitta relevanta artiklar. Artiklarna granskades och resulterade i totalt 14 stycken som användes i resultatet. Tre olika huvudteman urskildes i artiklarna och dessa var: Kroppsbild efter behandling, Sexuell dysfunktion efter behandling samt Behandlingsspecifika samband. RESULTAT: Resultatet visade att sexuell dysfunktion och kroppsbild påverkades negativt av både fysiska och psykiska faktorer så som dysparenui, minskad lust, blödningar och flytningar från vagina, rädsla att samlag skulle skada vagina efter behandling, förminskad och trång vagina. Behandlingsmetoden hade en stor betydelse för hur allvarliga dessa faktorer blev . SLUTSATS: Det är av stor vikt att som sjuksköterska och vårdpersonal uppmärksamma de problem och komplikationer som uppstår efter behandling av cervixcancer för att kunna bemöta patienterna och främja deras hälsa.

    Introduction:Among children and parents, the word surgery alone can be experienced as daunting. It may arouse feelings of anxiety, anxiety, anxiety and fear. The worries and anxieties can be reduced if parents and children are allowed to participate in the preoperative preparation. Purpose: The objective was to describe parents' experiences in connection with their child´s surgery and anesthesia. Method: The method was a systematic literature review. The data bases PsycINFO and Cinahl were used to retrieve articles based on parents´experiences and perceptions of their children´s surgery and anesthesia. The final number of articles included was 14 and the study design was both qualitative (n=6) and quantitative (n=8). Results: The results showed that information about the child's surgery and anesthesia could have a calming effect on the parents. It was very important that parents understood and could absorbed the information given. The hardest parts for the parents´ was the child´s anesthesia and that they had to leave the child behind in the operating room. Conclusion and clinicalimplications: Parents whose children are undergoing surgical care have a very great need for information about what will happen to their child. They should be treated in a professional manner as this can have significant impact on how they experience the care given, thus this is important to acknowledge in clinical care.

    Football (soccer) is endorsed as a health-promoting physical activity worldwide. When football programs are introduced as part of general health promotion programs, equal access and limitation of pre-participation disparities with regard to injury risk are important. The aim of this study was to explore if disparity with regard to parents' educational level, player body mass index (BMI), and self-reported health are determinants of football injury in community-based football programs, separately or in interaction with age or gender.

    Methodology/Principal Findings

    Four community football clubs with 1230 youth players agreed to participate in the cross-sectional study during the 2006 season. The study constructs (parents' educational level, player BMI, and self-reported health) were operationalized into questionnaire items. The 1-year prevalence of football injury was defined as the primary outcome measure. Data were collected via a postal survey and analyzed using a series of hierarchical statistical computations investigating associations with the primary outcome measure and interactions between the study variables. The survey was returned by 827 (67.2%) youth players. The 1-year injury prevalence increased with age. For youths with parents with higher formal education, boys reported more injuries and girls reported fewer injuries than expected; for youths with lower educated parents there was a tendency towards the opposite pattern. Youths reporting injuries had higher standardized BMI compared with youths not reporting injuries. Children not reporting full health were slightly overrepresented among those reporting injuries and underrepresented for those reporting no injury.

    Conclusion

    Pre-participation disparities in terms of parents' educational level, through interaction with gender, BMI, and self-reported general health are associated with increased injury risk in community-based youth football. When introduced as a general health promotion, football associations should adjust community-based youth programs to accommodate children and adolescents with increased pre-participation injury risk.

    Introduktion: Hjärtsvikt är ett tillstånd med påverkan på patienters livskvalitet ur ett multidimensionellt perspektiv. Det är den vanligaste orsaken till sjukhusinläggningar för patienter över 65 år. Livskvalitet är ett brett begrepp som innefattar en persons subjektiva upplevelse av sin livssituation. Syfte: Att identifiera och belysa vilka faktorer som påverkar upplevelsen av livskvalitet hos patienter som lever med kronisk hjärtsvikt. Metod: Denna litteraturstudie grundar sig på Polit och Becks (2012) niostegsmodell. Litteratursökning genomfördes i databaserna CINAHL och PubMed. En kvalitetsgranskning utfördes enligt Polit och Becks (2012) granskningsmallar, vilket resulterade i tolv vetenskapliga artiklar. Ur dataanalysen framträdde tre huvudkategorier. Resultat: De tre huvudkategorierna, fysiska faktorer, emotionella faktorer och sociala faktorer, består av de olika faktorer som på något vis påverkar livskvaliteten hos patienter med hjärtsvikt. Slutsats: Det finns ett flertal faktorer som påverkar upplevelsen av livskvalitet hos hjärtsviktspatienter.     Abstract [en]

    Objective

    The objective was to investigate relationships between adolescent sleep disturbance and various worry themes.

    Methods

    Questionnaire data from 8 cross-sectional collections between 1988 and 2011 were used. The sample included more than 20,000 adolescents aged 15-16. Binary logistic regressions were used for the analyses.

    Results

    Sleep disturbance and female sex increased the odds of worrying about all themes. Sleep disturbance shared stronger associations with worry about financial security, accidents/illness, being bullied, and terrorist attacks (odds ratios, 2.65-3.35) compared with worry about environmental destruction or nuclear war (odds ratios, 1.73-2.11). No interactions between sleep and year of investigation were found.

    Conclusions

    Little is known about the association between adolescent worry and sleep, and about sleep disturbance and specific worry content. This study shows that the strength in the relationship between adolescent worry and sleep varies with worry themes. Knowledge of the worry content related to sleep may aid in targeting preventions and interventions.

    The present study aimed to examine associations between the quality of the coach-athlete relationship and athlete exhaustion by assessing physiological and cognitive consequences. Male and female athletes (N = 82) representing seven teams across four different sports, participated in a quasi-experimental study measuring physical performance on a 5-meter multiple shuttle test, followed by a Stroop test to assess cognitive performance. Participants provided saliva samples measuring cortisol as a biomarker of acute stress response and completed questionnaires measuring exhaustion, and coach-athlete relationship quality. Structural equation modelling revealed a positive relationship between the quality of the coach-athlete relationship and Stroop performance, and negative relationships between the quality of the coach-athlete relationship and cortisol responses to high-intensity exercise, cognitive testing, and exhaustion. The study supports previous research on socio-cognitive correlates of athlete exhaustion by highlighting associations with the quality of the coach-athlete relationship. © 2018 Informa UK Limited, trading as Taylor & Francis Group

    Abstract [en]

    Traditionally, cancer patient follow-up has focused on disease surveillance and detecting recurrence. However, an increasing number of patients who have survived cancer acknowledge the importance of cancer rehabilitation issues and the need for more patient-oriented models of care by reporting their unmet physical, emotional, and social needs. Nurse-led follow-up care for cancer patients fulfills this need and has been developing gradually for various cancer diagnoses and prognoses. A growing body of evidence suggests that these services provide high-quality care that is both safe and efficient. Furthermore, patients benefit from the continuity of care and easy access to support for their multitude of needs, provided by such organized care. In this paper, we review the literature published in the past 5 years regarding nurse-led follow-up care for cancer patients in order to provide input and opinion for future research, clinical practice development, and nursing leadership. We pay special attention to head and neck cancer patients, a group that has been largely understudied and hence underreported in the literature. These patients have specific needs with respect to information and education regarding their cancer and potential treatment side-effects as well as a particular need for long-term psychosocial support and practical advice.

    Introduktion: Sverige är idag ett mångkulturellt land där en stor del av befolkningen har utländsk bakgrund. Flera av dem skattar sin munhälsa sämre än personer födda i Sverige och besöker tandvården mer sällan, trots att tandvårdsbehov finns. Genom att beskriva vilka faktorer som påverkar upplevelsen av svensk tandvård hos individer med utländsk bakgrund kan förståelsen för hur den upplevs öka.

    Syfte: Att beskriva faktorer som påverkar upplevelsen av svensk tandvård hos individer med utländsk bakgrund.

    Metod: Kvalitativ studie med enskilda intervjuer i en stad i Mellansverige. Urvalet bestod av fyra vuxna informanter i åldrarna 25-50 år med utländsk bakgrund. Intervjuerna analyserades med kvalitativ innehållsanalys.

    Resultat: Fyra huvudkategorier framkom vilka var bemötande, kommunikation, tandvårdsorganisation och vårdkvalitet. Bemötandet upplevs som professionellt, dock förekommer vissa motsättningar i patientmötet. Kommunikationen mellan personal och patient upplevs som god men information på andra språk än svenska efterfrågas. Gällande tandvårdsorganisation anses svensk tandvård som dyr och tidskrävande. Den svenska tandvården håller dock en god vårdkvalitet.

    Konklusion: Bemötande, ekonomi, kommunikation och vårdkvalitet är faktorer som påverkar upplevelsen av svensk tandvård hos individer med utländsk bakgrund. I de flesta fall var upplevelsen positiv men informanterna framförde vissa brister.

    Background: Early life is suggested to be a critical time in determining subsequent asthma development, but the extent to which the effect of early-life exposure to ambient air pollution on childhood asthma is unclear. Objectives: We investigated doctor-diagnosed asthma in preschool children due to exposure to ambient air pollution in utero and during the first year of life. Methods: In total 2490 children aged 3-6 years participated in a questionnaire study regarding doctor-diagnosed asthma between September 2011 and January 2012 in China. Children's exposure to critical air pollutants, sulfur dioxide (SO2) as proxy of industrial air pollution, nitrogen dioxide (NO2) as proxy of traffic pollution, and particulate matter <= 10 mu m in diameter (PM10) as a mixture, was estimated from the concentrations measured at the ambient air quality monitoring stations by using an inverse distance weighted (IDW) method. Logistic regression analysis was employed to determine the relationship between early-life exposure and childhood asthma in terms of odds ratio (OR) and 95% confidence interval (CI). Results: Association between early-life exposure to air pollutants and childhood asthma was observed. SO2 and NO2 had significant associations with adjusted OR (95% CI) of 1.45 (1.02-2.07) and 1.74 (1.15-2.62) in utero and 1.62 (1.01-2.60) and 1.90 (1.20-3.00) during the first year for per 50 mu g/m(3) and 15 mu g/m(3) increase respectively. Exposure to the combined high level of SO2 and NO2 in China significantly elevated the asthmatic risk with adjusted OR (95% CI) of 1.76 (1.18-2.64) in utero and 1.85 (1.22-2.79) during the first year compared to the low level exposure. The associations were higher for males and the younger children aged 3-4 than females and the older children aged 5-6. Conclusions: Early-life exposure to ambient air pollution is associated with childhood asthma during which the level and source of air pollution play important roles. The high level and nature of combined industrial and traffic air pollution in China may contribute to the recent rapid increase of childhood asthma. (C) 2015 Elsevier Inc. All rights reserved.

    Background: Bisphenols and triclosan are considered as potential thyroid disruptors. While mild alterations in maternal thyroid function can result in adverse pregnancy and child developmental outcomes, there is still uncertainty whether bisphenols or triclosan can interfere with thyroid function during pregnancy. Objectives: We aimed to investigate the association of urinary bisphenol A (BPA), bisphenol S (BPS), bisphenol F (BPF) and triclosan with early pregnancy thyroid function. Methods: This study was embedded in the Swedish Environmental Longitudinal, Mother and child, Asthma and allergy study (SELMA), a population-based prospective pregnancy cohort. In total, 1996 participants were included in the current study. Maternal urinary concentrations of three bisphenols and triclosan, collected at median (95% range) 10 (6–14) weeks of pregnancy as well as serum concentrations of thyroid stimulating hormone (TSH), free thyroxine (FT4), free triiodothyronine (FT3), total thyroxine (TT4), and total triiodothyronine (TT3) were measured. Results: Higher BPA levels were associated with lower TT4 concentrations (non-monotonic, P = 0.03), a lower FT4/FT3 ratio (β [SE] -0.02 [0.01], P = 0.03) and a lower TT4/TT3 ratio (β [SE] -0.73 [0.27], P = 0.008). Higher BPF levels were associated with a higher FT3 (β [SE] 0.01 [0.007], P = 0.04). There were no associations between other bisphenols or triclosan and absolute TSH, (F)T4 or (F)T3 concentrations. The association of BPA with thyroid function differed with gestational age. The negative association of BPA with FT4/FT3 and TT4/TT3 ratios was only apparent in early but not late gestation (P for interaction: 0.003, 0.008, respectively). Conclusion: These human data during pregnancy substantiate experimental findings suggesting that BPA could potentially affect thyroid function and deiodinase activities in early gestation.

    Context: Establishing reference ranges as well as identifying and quantifying the determinants of thyroid function during pregnancy is important for proper clinical interpretation and optimizing research efforts. However, such data are sparse, specifically for triiodothyronine measurements, and most studies do not take into account thyroid antibodies or human chorionic gonadotropin. Objective: To determine reference ranges and to identify/quantify determinants of TSH, free T4 (FT4), free triiodothyronine (FT3), total T4 (TT4), and total triiodothyronine (TT3). Design, Setting, and Participants: This study included 2314 participants of the Swedish Environmental Longitudinal, Mother and child, Asthma and allergy study, a population-based prospective pregnancy cohort of mother-child pairs. Reference ranges were calculated by 2.5th to 97.5th percentiles after excluding thyroperoxidase antibody (TPOAb)-positive and/or thyroglobulin antibody (TgAb)-positive women. Intervention: None. Main Outcome Measures: TSH, FT4, FT3, TT4, and TT3 in prenatal serum. Results: After exclusion of TPOAb-positive women, reference ranges were as follows: TSH, 0.11 to 3.48 mU/L; FT4, 11.6 to 19.4 pmol/L; FT3, 3.72 to 5.92 pg/mL; TT4, 82.4 to 166.2 pmol/L; and TT3, 1.28 to 2.92 nmol/L. Additional exclusion of TgAb-positive women did not change the reference ranges substantially. Exposure to tobacco smoke, as assessed by questionnaires and serum cotinine, was associated with lower TSH and higher FT3 and TT3. Body mass index (BMI) and gestational age were the main determinants of TSH (only for BMI), FT4, FT3, TT4, and TT3. Conclusions: We show that the exclusion of TgAb-positive women on top of excluding TPOAb-positive women hardly affects clinical reference ranges. We identified various relevant clinical determinants of TSH, FT4, FT3, TT4, and TT3 that could reflect endocrine-disrupting effects and/or effects on thyroid hormone transport or deiodination.
