    Abstract [en]

    Most studies concerning older people’s health and well-being have focused on their ill health, disease and complaints and have mainly been conducted with a quantitative design. Hence, there is still a need for qualitative studies in which these peoples’ own views on health and well-being are shown, especially when they are dependent on health care in a community setting. A qualitative approach was used in the study reported here and aimed at obtaining a deeper understanding of older peoples’ own views about their health and well-being. Interviews were carried out on two occasions with 19 older people living in their own homes and in sheltered accommodation. The data were analysed using content analysis. The findings suggested that the possibility to feel healthy was dependent both on the older person’s own ability to adjust or compensate to their situation, and on how their caregivers, relatives and friends could compensate for the obstacles the older person faced. The subcategories that captured the informants’ experiences of health and ill health were described as positive and negative poles of autonomy, togetherness, tranquillity and security in daily life. The significance of the caregivers was clearly evident. Their competence, commitment and treatment were prerequisites for the older person’s ability to experience health in spite of being dependent on care.

    Abstract [en]

    In spite of a considerable body of research in the past decades on what does or does not constitute good care for older people, there are still few studies addressing this question in which older people narrate their experiences of being dependent on community care. This study was therefore carried out aiming to explore older people's lived experiences of what good and bad care meant to them, when it was offered by community care services. Nineteen older persons in three Swedish communities participated in the study, which used a phenomenological-hermeneutic approach. Data were collected through unstructured interviews and Colaizzi's framework was utilized in the analysis of the data.The key theme arising from the analysis was that of being encountered as a human being by caregivers who, through the provision of safe and secure care, provide opportunities for living life as usual. When any of these circumstances are lacking, bad care will be the consequence. As the general intention in society is to ensure good quality of care to older people as well as others, the findings in our study should have important implications for providers of community care for older people.

    Abstract [en]

    The appearance of new anti-epileptic drugs (AED) during the last decade has provided neurologists and their patients with a greater choice, but the proof for their superiority over traditional AEDs is sparse, especially their use in adolescence and young adulthood. We studied a group of young adults (18–27 years) with epilepsy and compared their situation in 2004 with those 5 years earlier.

    Materials and methodsThe participants (n = 97) answered questionnaires regarding seizure-frequency, AED, side-effects and quality-of-life. Information was also taken from medical records.

    ResultsThe use of new generation AEDs increased during the 5-year study period, particularly among women. However seizure frequency had not changed significantly over time, and compared to men the effectiveness in controlling seizures was lower in women. The participants reported normal quality-of-life (QOL), which may indicate that the increase in number of AEDs to choose from actually improved the situation for these young adults with epilepsy. Frequency of seizures and cognitive side-effects of AEDs were associated with a lower QOL.

    ConclusionsMore women than men seem to be treated with new AEDs, and that the increase in use of new AEDs does not reduce seizure frequency in young adulthood. The effectiveness in controlling seizures seems to be lower in women in the age group studied. Further studies are required to better understand how epilepsy related factors interact.

    Abstract [sv]

    Licentiatavhandlingens övergripande syfte var att studera konsekvenser av ökad akademisering och akademiskt lärande inom Svensk specialist-sjuksköterskeutbildning efter genomförande av ny utbildningsreform enligt Bologna.

    Bakgrund: Specialistsjuksköterskor förväntas i dagens hälso– och sjukvård att kunna bedriva en patientsäker och evidensbaserad vård, vilket kräver akademisk kompetens. Utbildningen till specialistsjuksköterska fördes i och med Bolognaprocessens införande i Sverige 2007 till avancerad utbildningsnivå. Både yrkesexamen och akademisk examen (magisterexamen) skulle inkluderas och integreras i samma utbildning. Detta medförde en förändring från en ämnesinriktad (samlingskodsinriktad) läroplan till en läroplan som syftar till att integrera ämnen (integrationskodsinriktad). Förutom ämnesintegrering skulle yrkesspecifika mål integreras med akademiska mål.

    Metod: Artikel I genomfördes i form av ett didaktiskt aktionsforskningsprojekt som pågick under 2½ år. En projektledningsgrupp (n=15-18) från fem lärosäten bildades. Denna grupp har aktivt deltagit i hela aktionsforskningsprocessen och vid projektmöten. Vid varje lärosäte utsågs en projektansvarig som ansvarade för den lokala aktionsforskningsprocessen. Dessa personer samlande in data i form av kursplaner, utbildningsplaner, studiehandledningar, minnesanteckningar från lärarmöten och möten med vårdverksamheter samt minnesanteckningar från forskargruppensmöten. Analys har gjorts med hjälp av konstant komparativ analysmetod. I artikel II användes konstruktivistisk Grounded Theory för att samla in och analysera data. Datainsamlingen påbörjades utifrån specialistsjuksköterskestudenters svar på en enkäts öppna fråga (n=120) i samband med utbildningens avslutning 2009 och 2010. Enkätsvaren reste frågor om specialistsjuksköterskans akademiska lärande. För att inhämta en djupare förståelse genomfördes intervjuer med specialistsjuksköterskor (n=12) som yrkesarbetat mellan 5 och 12 månader efter sin examen. Intervjuerna genomfördes från juni till oktober 2011. Avslutningsvis jämfördes svaren på den öppna enkätfrågan med analysen av intervjuerna.

    Resultat: Vid utbildningsreformens införande fanns pedagogiska motsättningar och didaktiska svårigheter som gav upphov till spänningsförhållanden mellan vårdutveckling, forskning, yrkesspecifika mål och akademiska mål. Spänningsförhållanden framkom också när specialistsjuksköterskorna inte erfor att deras akademiska kompetens tillvaratogs och värderades i vårdverksamheten. När stöd inte fanns för akademiskt lärande uppkom villrådighet och specialistsjuksköterskorna blev ambivalenta över om akademiskt lärande var meningsfullt. För att främja meningsfullt akademiskt lärande framhåller deltagarna betydelsen av samverkan och gemensamma arenor mellan lärosäten och vårdverksamhet där nyttoaspekten med akademiskt lärande tydliggörs. De spänningsförhållanden som framkommer var av både positiv och negativ karaktär.

    In conjunction with the introduction of the Bologna process in Sweden, specialist nursing education programmes were moved up to the second cycle of higher education with the opportunity to take a one-year master's degree, which also meant that students would undertake a degree project carrying 15 ECTS. Thepurpose of this study was to examine the introduction of postgraduate degree projects on the second-cyclelevel into Swedish specialist nursing programmes in accordance with the Bologna process. Five universities were involved and the study design took the form of action research.

    Problem formulation, planning, evaluation and follow-up with reflection led to new actions over a period of 2 1/2 years. Through a review oflocal curriculum documents, the implementation of a postgraduate degree project was monitored and thesereviews, together with field notes, were analysed by means of constant comparative analysis. The results revealed a variety of tensions that arose when postgraduate degree projects were introduced, taking the form of differing views on the relationship between research, clinical development, specific professional objectives and academic objectives. These tensions were reflected in six areas of change. In summary, it can be noted that implementation of the postgraduate degree projects highlighted tensions related to basic views of learning.

    Introduktion

    Livskvalitet är ett subjektivt begrepp och det finns flera olika definitioner Merparten definierar det som en patients tillfredsställelse av någon del av livet. Ett av de viktigaste målen med palliativ vård i livets slutskede är att främja livskvalitet hos patienter.

     

    Syfte

    Syftet med litteraturstudien var att beskriva omvårdnad som främjar livskvaliteten i livets slutskede inom palliativ vård, ur ett patientperspektiv.

     

    Metod

    Litteraturstudien följde de nio stegen enligt Polit & Becks modell (2012). Artiklarna söktes i databaserna CINAHL, PubMed och PsycInfo. Efter en granskning av kvaliteten resulterade det i tio artiklar. Tre kvantitativa artiklar och sju kvalitativa artiklar genomgick tre urval och kvalitetsgranskades. Med litteraturstudiens syfte i fokus framkom det olika mönster som bildade sex teman.

     

    Resultat

    De olika teman som framkom var: Symtomlindring, främst smärtlindring. Känslan av självbestämmande, att få möjligheten att påverka sin vård. Sociala relationer, familj och vänner hade en stor betydelse. Känsla av trygghet/säkerhet, att känna tilltro till vården. Andliga relationer och förberedelser inför döden, tro till en högre makt. Positiv inställning till livet, att tänka positivt.

     

    Slutsats

    Smärtlindring och sociala relationer var två framstående delar som främjade livskvalitet i omvårdnad i livets slutskede. Eftersom livskvalitet är ett subjektivt begrepp och individanpassat var även självbestämmande av vikt för patienten.

    Introduktion: Hunden har kallats "människans bästa vän" kanske är den också det. Vårdhunden har blivit vanligare på såväl sjukhus som i kommunal vårdmiljö och kan vara ett icke-farmakologiskt komplement i omvårdnaden för personer med demenssjukdom.Syftet: Att granska vilken inverkan vårdhunden har för personer med demenssjukdom.Metod: Litteraturstudie i 9-steg enligt Polit & Beck (2012) med datainsamling i databaserna PsycINFO, PubMed och CINAHL. Kvalitetsgranskning av artiklarna gjordes enligt Polit & Beck (2012) med granskningsmall för kvalitativa och kvantitativa studier. De artiklar som kvarstod i urval 3 var 11till antalet och dessa presenterades i tre teman.Resultat: De teman som framträdde var, Själslig påverkan Relationell påverkan och Kroppslig påverkan.Slutsats: Vårdhund har en relativt kortvarig men signifikant inverkan på livskvaliteten för personer med demenssjukdom, samt i omvårdnaden av dessa. Det behövs ytterligare forskning inom området och den personcentreradevård som möjliggörs genom vårdhundmöten. Detta då icke- farmalogiska alternativ för personer med demenssjukdom är ett åsidosatt om än så viktigt område att bedriva forskning inom.

    Abstract [en]

    Background: Extensive studies have shown that older people are negatively impacted by impaired eating and nutrition. The abilities to eat, enjoy food, and participate in social activities associated with meals are important aspects of health-related quality of life (HRQoL) and recovery after illness. This project aims to (i) describe and analyze relationships between oral health and oral HRQoL, swallowing ability, eating ability, and nutritional risk among older individuals admitted to short-term care; (ii) compare the perceptions that older individuals and staff report on care quality related to oral hygiene and eating; and (iii) study the feasibility and effects of a training program for people with impaired swallowing (i.e., dysphagia). Methods/Design: This project consists of two parts, which will be performed in five Swedish counties. It will include approximately 400 older individuals and 200 healthcare professionals. Part 1 is a cross-sectional, descriptive study of older people admitted to short-term care. Subjects will be assessed by trained professionals regarding oral health status, oral HRQoL, eating and nutritional risk, and swallowing ability. Swallowing ability will be measured with a teaspoon test and a swallowing capacity test (SCT). Furthermore, subjects and staff will complete a questionnaire regarding their perceptions of care quality. Part 2 is a cluster randomized intervention trial with controls. Older participants with dysphagia (i.e., SCT < 10 ml/s, measured in part 1) will be recruited consecutively to either the intervention or control group, depending on where they were admitted for short-term care. At baseline, all subjects will be assessed for oral health status, oral HRQoL, eating and nutritional risk, swallowing ability, and swallowing-related QoL. Then, the intervention group will receive 5 weeks of training with an oral screen for neuromuscular training focused on orofacial and pharyngeal muscles. After completing the intervention, and at six months post-intervention, all assessments will be repeated in both study groups.     Abstract [sv]

    Introduktion Personcentrerad vård innebär att omvårdnaden bedrivs ur ett holistiskt perspektiv där hela människan synliggörs. Fokus ligger inte enbart på de fysiska behoven, utan psykiska, existentiella, sociala och andliga behov får ta lika stor plats. Syfte Att belysa sjuksköterskans erfarenheter av vad som är viktigt för att kunna ge en personcentrerad vård, inom somatisk vård. Metod En litteraturstudie gjordes, med induktiv ansats, enligt Polit & Becks (2017) 9 steg. Databassökning i Cinahl och PubMed resulterade i 10 artiklar. Resultat De teman som framkom ur sammanställningen av artiklarna var att individanpassa vården, att göra patienten delaktig och att implementera ett personcentrerat synsätt på arbetsplatsen. Sjuksköterskorna har många positiva erfarenheter av personcentrerad vård. De anser att det är viktigt att lyssna till patientens berättelse och uppnå ett partnerskap där patienten blir en del av teamet. Det finns dock hinder för den personcentrerade vården, såsom dåligt med tid och otillräckliga resurser. Slutsats Genom att individanpassa vården och göra patienten delaktig i sin egen vård med ett personcentrerat ledarskap och teamwork i fokus, kan sjuksköterskorna inte bara göra skillnad för patienterna och främja en kostnadseffektiv vård, utan även skapa arbetsglädje och välmående för hela vårdteamet.
