    Patients’ perceptions of actual care conditions and patient satisfaction with care quality in hospital2012Doctoral thesis, comprehensive summary (Other academic)
    Abstract [en]

    There are theoretical and methodological difficulties in measuring the concepts of quality of care and patient satisfaction, and the conditions associated with these concepts. A theoretical framework of patient satisfaction and a theoretical model of quality of care have been used as the theoretical basis in this thesis.

    Aim. The overall aim was to describe and explore relationships between person-related conditions, external objective care conditions, patients’ perceptions of quality of care, and patient satisfaction with care in hospital.

    Methods. Quantitative and qualitative methods were used. In the quantitative study (I-III), 528 patients (83.7%) from eight medical, three surgical and one mixed medical/surgical ward in five hospitals in Norway agreed to participate (10% of total discharges). Data collection was conducted using a questionnaire comprising four instruments: Quality from Patients’ Perspective (QPP); Sense of Coherence scale (SOC); Big Five personality traits – the Single-Item Measures of Personality (SIMP); and Emotional Stress Reaction Questionnaire (ESRQ). In addition, questions regarding socio-demographic data and health conditions were asked, and data from ward statistics were included. Multivariate statistical analysis was carried out (I-III). In the qualitative study 22 informants were interviewed (IV). The interviews were analysed by conventional content analysis.

    Main findings. Patients’ perceptions of quality of care and patient satisfaction ranged from lower to higher depending on whether all patients or groups of patients were studied. The combination of person-related and external objective care conditions explained 55% of patients’ perceptions of quality of care (I). 54.7% of the variance in patient satisfaction was explained, and the person-related conditions had the strongest impact, explaining 51.7% (II). Three clusters of patients were identified regarding their scores on patient satisfaction and patients’ perceptions of quality of care (III). One group consisted of patients who were most satisfied and had the best perceptions of quality of care, a second group of patients who were less satisfied and had better perceptions, and a third group of patients who were less satisfied and had the worst perceptions. The qualitative study revealed four categories of importance for patients’ satisfaction: desire to regain health, need to be met in a professional way as a unique person, perspective on life, and need to have balance between privacy and companionship (IV).

    Conclusions. Patients’ perceptions of quality of care and patient satisfaction are two different concepts. The person-related conditions seem to be the strongest predictors of patients’ perceptions of quality of care and patient satisfaction. Registered nurses need to be aware of this when planning and conducting nursing care. There is a need of guidelines for handling over‑occupancy, and of procedures for emergency admissions on the wards. The number of registered nurses on the wards needs to be considered. Healthcare personnel must do their utmost to provide the patients with person‑centred care.

    Abstract [en]

    Aims and objectives. To describe patients' perceptions of quality of care and to explore combinations of person-related and external objective care conditions as potential predictors of these perceptions. Background. Several studies have examined various single factors of person-related and external objective care conditions in relation to quality of care. None of these has included the effect of over-occupancy on patients' perception of quality of care. Furthermore, little is known about how combinations of different factors are related to each other and to the perception of quality of care using multivariate analysis. Design. A cross-sectional design. Method. A total of 528 patients (83·7%) from 12 medical, surgical or medical-surgical wards in five hospitals in Norway participated. Perceptions of quality of care and person-related conditions were measured with the 'Quality from Patient's Perspective' instrument. Data on external objective care conditions was collected from ward statistics provided by head nurses. Multivariate general linear modelling was used ( p < 0·05). Results. The combination of person-related and external objective care conditions revealed five factors that predict patients' perception of quality of care. Three of these are person-related conditions: sex, age and self-reported psychological well-being and two of them are external objective care conditions: RNs (headcount) on the wards and frequency of over-occupancy. These five factors explained 55% of the model. Patients rated the quality of care high. Conclusions. Sex, age, psychological well-being, frequency of over-occupancy and the number of RNs are important factors that must be emphasised if patients are to perceive the quality of care as high. Relevance to clinical practice. Head nurses and healthcare authorities must continually prepare the wards for over-occupancy and they must consider the number of RNs working on the wards.

    Abstract [en]

    Rationale, aim and objectives: Patients experiencing high care quality and satisfied patients are more likely to follow treatments. Patient satisfaction is an important contributor to physical and mental health-related quality of life. Research emphasises the need to further study satisfaction from the patients’ perspective. The aim was to describe patients’ experiences of care quality and the relation to their satisfaction during hospital stay. 

    Methods: A qualitative descriptive design was used. Interviews were conducted with 22 patients discharged from hospital. Data was analysed by conventional content analysis.

    Results: Four categories and seven subcategories describing patients’ experiences of care quality and the relation to their satisfaction emerged. Desire to regain health comprised waiting for treatment, being cured, having hopes of being cured and described the treatment and health outcome of hospitalisation. Need to be met in a professional way as a unique person comprised receiving personalized knowledge, receiving healthcare by competent healthcare personnel and described the way patients need to be met by healthcare personnel. Need to be involved comprised taking responsibility for own health, leaving responsibility for own health and concerned the patients’ way of handling hospitalisation. Need to have balance between privacy and companionship concerned the relationship to fellow patients.

    Conclusions: Health condition is of great importance to patients’ experiences of quality of care and their satisfaction in relation to hospital stay. The healthcare personnel need to be aware that seriously ill patients may never be completely satisfied. Furthermore, healthcare personnel must do their utmost to provide the patients with person-centered care.  Hospital managers must consider the design of wards with respect to such matters as multiple-bed versus single-bed rooms and heads of nursing must carefully plan each patient’s accommodation.

    Abstract [en]

    Aim. This study aimed at evaluating the outcome of implemented evidence-based clinical guidelines by means of surveying the frequency of thrombophlebitis, nurses’ care, handling and documentation of peripheral intravenous cannulae. Background. Peripheral intravenous cannulae are frequently used for vascular access and, thereby, the patients will be exposed to local and systemic infectious complications. Evidence-based knowledge of how to prevent these complications and how to care for patients with peripheral intravenous cannula is therefore of great importance. Deficient care, handling and documentation of peripheral intravenous cannulae have previously been reported. Design. A cross-sectional survey was conducted by a group of nurses at three wards at a university hospital before and after the implementation of the evidence-based guidelines. Method. A structured observation protocol was used to review the frequency of thrombophlebitis, the nurses’ care, handling and the documentation of peripheral intravenous cannulae in the patient's record. Results. A total of 107 and 99 cannulae respectively were observed before and after the implementation of the guidelines. The frequency of peripheral intravenous cannulae without signs of thrombophlebitis increased by 21% ( P < 0·01) and the use of cannula size 0·8 mm increased by 22% ( P < 0·001). Nurses’ documentation of peripheral intravenous cannula improved significantly ( P < 0·001). Conclusion. We conclude that implementation of the guidelines resulted in significant improvements by means of decreased frequency of signs of thrombophlebitis, increased application of smaller cannula size (0·8 mm), as well as of the nurses’ documentation in the patient's record. Relevance to clinical practice. Further efforts to ameliorate care and handling of peripheral intravenous cannulae are needed. This can be done by means of increasing nurses’ knowledge and recurrent quality reviews. Well-informed patients can also be more involved in the care than is common today.

    Abstract [en]

    Introduction: District nurses meet informal caregivers of different ages, family constellations and in diverse contexts such as health care centers, child health care centre, home care and school health care. In the home, informal caregivers care a large part of the care provider's nursing and healthcare. In addition to family members, there may also be friends who are close to the care recipient. The aim: To describe the district nurse's experiences of caring together with informal caregivers. Method: The method used was a literature study with manifest content analysis. 14 articles were included in the essay analysis. Results: Six categories emerged: building relationships (subcategory: trust and trust), good communication, cooperation (subcategory: involvement of close relatives), support and accessibility, lack of communication (subcategory: conflicts of interests), factors in the family. These categories are summarized in two themes: informal caregivers as resource and informal caregivers as a burden. Conclusion: The results of the study show that the district nurse's experience of caring with informal caregivers is that it can both be a resource and a burden. The district nurses experience is that informal caregivers are a resource if they are included. The results show that the relationship between the nurse and informal caregivers is important for the cooperation and planning of the patient's care in the absence of the district nurse. District nurses need to raise awareness for the encounter with informal caregivers and patients who have other cultural beliefs and lifestyles than their own.

    Introduktion: Hypertoni är en global folksjukdom som leder till stora kostnader för samhället och lidande för individen. Läkemedel och livsstilsförändringar utgör behandlingen. Sjuksköterskan har en central roll i hypertonibehandling, studier visar dock att följsamhet till behandling är låg. Litteratursammanställning belyser vilka interventioner sjuksköterskan kan använda sig av för att förbättra patienters följsamhet till hypertonibehandling.

    Syfte: Syftet med litteratursammanställningen var att belysa interventioner i relation till följsamhet hos patienter med hypertonibehandling. Utifrån problemformuleringen och syftet formulerades två frågeställningar: 1. Vilka interventioner finns beskrivna? 2. Ger de effekt? 

    Metod: Litteratursammanställningen genomfördes enligt Polit och Becks (2017) nio steg. Sökningarna utfördes i databaserna Cinahl och Pubmed och resulterade i sammanlagt tio artiklar, nio av kvantitativ design och en med kvalitativ design. Samtliga artiklar granskades enligt Polit och Becks (2017) granskningsmallar. Databearbetningen utfördes genom tematisk analys.

    Resultat: Tre teman framträdde: minnesinterventioner, motiverande interventioner och utbildande interventioner. I temat utbildning framträdde två subteman: grupputbildning och individuell utbildning. Alla interventioner som innehöll en motiverande komponent gav signifikant förbättrad följsamhet till antingen läkemedelsbehandling, livsstilsbehandling eller både och.

    Slutsats: En motiverande komponent är en nyckelaspekt för sjuksköterskan att ta i beaktande i arbetet för förbättrad följsamhet till hypertonibehandling.

    Andelen äldre personer kommer i framtiden att öka och behovet av en god och värdig vård blir en utmaning inom den kommunala äldreomsorgen. I samband med att vård utvärderas relateras det ofta till bevarad eller kränkt värdighet. Begreppet värdighet är ett grundbegrepp inom omvårdnad som är något fundamentalt och självklart hos alla människor. Upplevelsen av bevarad värdighet är beroende av i vilken utsträckning viktiga värden kan uppnås. Media skriver om att kränkningar förekommer i vården av äldre som bor och lever på äldreboende. I vilken mån upplevelsen av värdighet kan återfås efter en kränkning är beroende av om en försoning med situationen kan ske. Avsikten med studien var att utifrån litteratur beskriva innebörden av begreppet bevarad värdighet och vidga förståelsen för den äldre personens erfarenheter av vård och hjälp samt hur livet förändras på äldreboende. I de två delstudierna framkommer nya dimensioner av begreppet bevarad värdighet och när vårdaren är inkännande och lyssnar till den äldres berättelse framträder bevarad värdighet. I beskrivningen av hur äldre personer erfar att vara i behov av vård och hjälp de sista levnadsåren synliggörs att de strävar efter att behålla en inre känsla av stolthet. Vården bör vara planerad och utformad så att den äldre kan komma tillrätta när kroppen och livssituation förändras och de blir i behov av vård och omsorg.

    Elderly persons' participation in and decisions about their own care need more attention. The aim of this study was to gain a deeper understanding of elderly persons' experiences of care and help, and how their lives change in nursing homes. Fifteen elderly persons living in four nursing homes were interviewed about their experiences of needing care and help in their daily life. The interviews were analysed using van Manen's phenomenological approach. Elderly persons' experiences of care and help were described as: a balance between sorrow and relief, a struggle to maintain control and connectedness, managing to live in the present and yet worrying about the future, and an attempt to hide one's vulnerability in order to be accepted and create an inner calm in an exposed situation. In conclusion, important issues were raised concerning elderly persons' perspectives on care and help in daily life in nursing homes.

    Syftet var att belysa patienters upplevelser av att ha medicinska larver som sårvårdsbehandling vid svårläkta sår.

    Metoden som valts och använts för genomförande var litteraturstudie med litteratursökningar från databaserna CINAHL och PubMed. Granskning av artiklar och databearbetning har skett utifrån Polit och Becks niostegsmodell för litteraturstudier.

    Resultatet är skrivet med stöd från tio artiklar, varav fyra är kvalitativa och sex är kvantitativa. Resultatet är indelat i tio kategorier under två huvudkategorier. Kategorierna smärta, klåda, kittlande, stickningar, förnimmelse, exsudat och lukt har samlats under huvudkategorin sårrelaterade upplevelser. Kategorierna obehag, attityder och närståendes rektioner har samlats under huvudkategorin känslomässiga upplevelser. Huvudresultatet från studien är att sårdebridering med larver ur ett patientperspektiv på ett relativt smärtfritt sätt kan ha gynnsamma effekter för patienten med bland annat minskad sårlukt och sårvätska. Det är viktigt med god patientinformation inför behandling med larver på grund av attityder hos patienter mot behandlingen.

    BACKGROUND: Suffering a myocardial infarction (MI) is a life-threatening event that impacts not only on the individual concerned but also on the next of kin. However, there seems to be a paucity of naturalistic inquiries that focus specifically on midlife next of kin and their experience of being close to a relative who has suffered an MI. This study aims to elucidate the experience of being a midlife next of kin of a relative who has suffered a myocardial infarction. METHOD: Nine women and four men in midlife participated in the focused interviews, which were conducted and analysed during 2010/2011 using Lindseths and Norbergs' description of the phenomenological hermeneutical method. FINDINGS: Four themes - Solely responsible, Lurking unease, Left out of the picture and Life on hold - formed the basis of the core theme Standing alone when life takes an unexpected turn. The core theme was interpreted as a central phenomenon encompassing the experience of being solely responsible for the well-being of their relative and the family, thus putting their own life on hold. The core theme also reflected the next of kin's experience of being left out of the picture when it came to the relative's care before and after the MI. CONCLUSION: The next of kin's negative feelings of standing alone were further intensified by their experience of being left out of the picture by the healthcare professionals concerning their relative's care. As a cardiac nurse, it would seem essential to have knowledge about the experiences of next of kin in connection with a relative's MI event. Such knowledge can facilitate the planning and organisation of nursing care and at the same time address the next of kin's role in the recovery and rehabilitation process.

    The aim of this study was to elucidate the meaning of the experience of younger people (< 55 years) during their first year following a myocardial infarction. We analyzed 17 interviews using a phenomenological-hermeneutic method. The core theme and central phenomenon was the everyday fight to redress the balance in life, which encompassed an existential, physical, and emotional battle to regain a foothold in daily life. The aftermath of a life-threatening event involved a process of transition while at the same time creating a new meaning in life. Lack of energy and its impact on the complex interplay of midlife combined with unreasonable demands from employers and health care professionals seemed to color the experience of the informants. The knowledge gained in this study can constitute a valuable contribution to overall quality assurance in nursing care and the development of nursing interventions for the cardiac rehabilitation of younger patients.

    Introduktionen: I Sverige brännskadas ca 20 000 människor om året.  Livskvalitet anses vara olika för olika individer. För att bedöma livskvalitet efter en brännskada används The Burn Specific Health Scale- Brief (BSHS- B) som är ett brännspecifikt bedömningsinstrument. Syfte: Syftet med litteraturstudien var att beskriva patienters  upplevelser av livskvalitet efter en brännskada när livskvalitet bedömts med hjälp av The Burn Specific Health Scale- Brief. Metod: En litteraturstudie genomfördes med hjälp av en modifierad niostegsmodell efter Polit & Beck (2012). Artiklarna granskades noggrant enskilt och tillsammans av författarna. Tio artiklar skulle forma resultatet. Resultat: Artiklarna analyserades utifrån domänerna Simple Abilities, Hand function, Work, Heat Sensitivity, Treatment Regimens, Body Image, Affect, Interpersonal Relationship och Sexuality som tillsammans bygger upp BSHS-B. Resultatets teman i denna litteraturstudie är uppbyggt med samma domäner. Mellan 2-7 år efter brännskadan skattades livskvaliteten som högre, det var få individer som hade svårigheter med handfunktionen och män hade något lättare att återgå till arbetet. Även värmekänsligheten skattades med stora svårigheter. Hälften av studierna visade att deltagarna hade en negativ kroppsuppfattning och att relationen till familjen inte var påverkad i så stor utsträckning. Sexualiteten hos män var mindre påverkad av brännskadan än hos kvinnor. Slutsats: Förmågan att arbeta och tiden efter brännskadan påverkar livskvaliteten. Kvinnor upplevde att sexualiteten, värmekänsligheten och arbetet påverkades mer negativt av brännskadan än vad män gjorde.

    Introduktion: Gynekologisk cancer är ett samlingsbegrepp för de cancerformer som drabbar kvinnans reproduktionsorgan. De olika cancerformerna har olika symtom och härkomst och därför kan behandlingen skiljas. Behandlingarna kan ge en rad olika biverkningar och konsekvenser vilket ger individuella erfarenheter. Syftet: Litteraturstudiens syfte var att belysa kvinnors erfarenheter av att leva med gynekologisk cancer. Metod: Litteraturstudie grundad på Polit och Becks (2012) niostegsmodell med induktiv ansats. Databaser som användes var CINAHL och PubMed. Artiklarna genomgick en kritisk granskning. Tio kvalitativa artiklar gick igenom kvalitetsgranskningen och svarade mot studiens syfte. Resultat: Resultatet delades in i tre huvudteman: leva med förändringar, inre kamp mot känslomässigt kaos och personlig utveckling. Resultatet visar att kvinnors erfarenheter baserades på de förändringar som de tvingades genomgå. Det innebar att kvinnorna behövde anpassa sig vilket var en större utmaning än de kunde föreställa sig. Slutsats: Litteraturstudiens resultat visade att kvinnor hade både negativa och positiva erfarenheter av gynekologisk cancer. De huvudsakliga erfarenheterna som kvinnorna upplevt var leva med förändringar, kämpa mot ett känslomässigt kaos och personlig utveckling.

    Introduktion: Ischemisk hjärtsjukdom är idag världens vanligaste dödsorsak vilket är en följd av olika metabola rubbningar inom metabolt syndrom. Samhällets utveckling har resulterat i minskad fysisk aktivitet, ökad stress och mindre hälsosam livsstil. I och med det har också risken ökat för utveckling av flera metabola rubbningar som exempelvis diabetes typ II, högt blodtryck och övervikt med metabolt syndrom som följd.

    En del av sjuksköterskans roll innebär att arbeta med identifiering och förebyggande av ohälsa, hälsofrämjande åtgärder, lindring av lidande och återställande av hälsa. Syftet med litteraturstudien var att belysa hur sjuksköterskan kan använda olika omvårdnadsåtgärder vid förebyggande av metabolt syndrom. Litteraturstudiens metod var utformad utefter Polit och Beck’s (2017) niostegsmodell. Databaserna CINAHL och PubMed användes som underlag där 14 kvantitativa artiklar valdes ut med efter induktivt förhållningssätt och med kvalitétsgranskning kopplade till litteraturstudiens syfte. Resultatet efter databearbetning och induktivt förhållningssätt bildade två huvudteman, utbildning och kontinuitet. Båda temana är aktuella och har betydelse för sjuksköterskan i det förebyggande arbetet av metabolt syndrom. Slutsatsen av utförd litteraturstudie var att sjuksköterskan kan bidra i det förebyggande arbetet hos individer som är drabbade av metabolt syndrom eller dess enskilda rubbingar. Sjuksköterskan kan stötta och vägleda mot en livsstilsförändring med hjälp av utbildning och återkommande kontakt med individen.

    Bakgrund: Distriktssköterskan har med sin breda kompetens en viktig roll inom primärvården. Distriktssköterskan besitter goda ämneskunskaper i omvårdnad, vårdpedagogik, medicin, och folkhälsovetenskap. Många länder i världen har valt att tillåta läkemedelsförskrivning till distriktssköterskor, vilket leder till en effektivare vård. Syfte: Litteraturstudiens syfte var att belysa distriktssköterskans erfarenhet av läkemedelsförskrivning inom primärvården. Metod: En systematisk litteraturstudie med kvalitativ induktiv design har genomförts. Till datainsamlingen gjordes systematisk litteratursökning i databaserna CINAHL och Pubmed. Totalt användes 11 artiklar som svarade på studiens syfte. Kvalitativ innehållsanalys enligt Graneheim och Lundman (2004) med manifest analys användes. Resultat: Dataanalysen mynnade ut i tre huvudkategorier. 1. Ökad självständighet och ansvar, 2. Förbättrad omvårdnad genom förskrivningsrätten med följande underkategorier: effektivare vård med kostnadseffektivitet och personcentrerad och holistisk vård. 3. Orsaker som påverkar hur förskrivningsrätten används i praxis med tre underkategorier: Viljan att hålla sig uppdaterad, behov av tillgång till stöd och begränsad förskrivning. Konklusion: Även om vården har förändrats har distriktssköterskans förskrivningsrätt inte utvecklats i samma utsträckning i Sverige som i andra länder. Litteraturstudiens resultat bekräftas av tidigare forskning att distriktssköterskornas läkemedelsförskrivning gav ökad självständighet, ökad status och ökad respekt bland kollegor och patienter. Resultatet visar att distriktssköterskorna önskar mer utbildning, fortbildning och träning för att öka kompetensen inom läkemedelsförskrivning.

    Bakgrund: Under 1970-talet ökade kraven på sjukvården där allmänheten ställde krav på ökad tillgänglighet såväl som vård för alla på lika villkor. Detta medförde en expandering av sjukvårdsrådgivning via alternativ kontakt, såsom över telefon. Där kunde sjuksköterskan arbeta med att hjälpa vårdtagaren till självhjälp genom egenvårdsråd och hänvisa vederbörande till rätt vårdnivå i ett tidigt stadium. Syfte: Att belysa faktorer som påverkar telefonsjuksköterskans upplevelse av att arbeta med rådgivning via telefon. Metod: En litteraturstudie baserad på Polit och Becks (2017) nio steg. CINAHL, PubMed samt PsycINFO användes för att samla in relevanta artiklar, de genomgick även kvalitetsgranskning enligt granskningsmallarna i Polit och Beck (2017). Induktiv ansats användes och resultatet bestod av tio artiklar. Resultat: Tre kategorier med bidragande faktorer som påverkade telefonsjuksköterskans upplevelse av arbetet framkom; - Kommunikation, beslutsfattande samt arbetsmiljö. Slutsats: Telefonsjuksköterskorna uppgav att flertalet faktorer påverkade deras upplevelse av arbetet och tryggheten i professionen. Dessa faktorer behövde dock inte bli ett problem om de blev hanterade på ett tillfredställande sätt.

    Introduktion: Att handleda studenter ingår i sjuksköterskans yrkesroll. Handledning utförs med en handledningsmodell som grund där samarbetet mellan lärosätet och den kliniska verksamheten tydliggörs. Olika pedagogiska modeller finns utarbetade med syfte att stödja studenten i lärandet. Kärnan i handledning är reflektion som möjliggör sammankoppling mellan teori och praktik. Att tydliggöra vilken betydelse reflektion har för studentens lärande är väsentligt.

      Syfte: Syftet med studien var att tydliggöra reflektionens betydelse för studentens lärande i klinisk praktik samt att beskriva faktorer som främjar respektive hindrar reflektion.

      Metod: Systematisk litteraturstudie valdes enligt Polit & Beck. Sökning som utfördes i databaserna CINAHL, PubMed och ERIC resulterade i 13 kvalitativa artiklar som ligger till grund för resultatet.

      Resultat: Resultatet tydliggjorde att reflektion främjade studentens lärande i klinisk praktik. Tydliga skillnader framkom gällande i vilken fas reflektion utövades. Reflektion efter handling var det som var mest förekommande. Reflektion före och under handling var ingen medveten strategi. Främjande faktorer var tillit och stöd och hindrande faktor som var mest framträdande var tidsbrist.

    Konklusion: När det skapas utrymme för reflektion i de olika faserna kan studenten koppla kliniska erfarenheter till teoretiska kunskaper och detta ger handlingsberedskap för nya situationer.
