Abstract [sv]

Covid-19-pandemin har setts kunna leda till förstärkta riskfaktorer i den psykosociala arbetsmiljön inom vården. Några riskfaktorer rapporteras sedan tidigare ha samband med samvetsstress, ett vanligt fenomen hos vårdpersonal. Resiliens tros vara viktigt för hur vårdpersonal hanterar och återhämtar sig från Covid-19-pandemin. Utifrån detta syftade föreliggande uppsats till att undersöka psykosocial arbetsmiljö, arbetsrelaterad self-efficacy i relation till samvetsstress, samt jämföra psykosocial arbetsmiljö hos vårdpersonal som vårdat patienter i livets slutskede under Covid-19-pandemin. Uppsatsen hade en tvärsnittsdesign. Data insamlades av PalliativeQualityCare-Covid19 under hösten 2021. 518 respondenter från olika vårdkontexter i Sverige besvarade enkäten. Jämfört med referensvärden visade resultatet på en bättre psykosocial arbetsmiljö avseende mening i arbetet, socialt stöd från kollegor och överordnad samt en sämre psykosocial arbetsmiljö avseende oro för förändrade arbetsförhållanden, självskattad hälsa, utbrändhet och stress. Jämförelse mellan omplacerad och icke-omplacerad personal visade inte på några skillnader inom psykosocial arbetsmiljö. Personal som vårdat patienter med konstaterad Covid-19 rapporterade sämre socialt stöd från överordnad än personal som inte vårdat patienter med konstaterad Covid-19. Signifikanta korrelationer erhölls mellan samvetsstress, arbetsrelaterad self-efficacy och de flesta dimensioner inom psykosocial arbetsmiljö. Psykosocial arbetsmiljö och arbetsrelaterad self-efficacy kunde förklara 54,2% av variansen i samvetsstress. Resultatet indikerar att psykosocial arbetsmiljö och arbetsrelaterad self-efficacy är betydande stressorer (risk- och friskfaktorer) i relation till samvetsstress hos vårdpersonal som vårdat patienter i livets slutskede under Covid-19-pandemin.

Abstract [en]

The Covid-19-pandemic has potentially resulted in reinforced risk factors within the psychosocial work environment in the healthcare field. Some risk factors have a connection to stress of conscience, a common phenomenon for health care workers. Resilience could be important for health care workers coping and recovering from the Covid-19-pandemic. This thesis examined the psychosocial work environment and organizational self-efficacy in relation to stress of conscience, additionally, comparing the psychosocial work environment, for health care workers caring for terminally ill patients during the Covid-19-pandemic. A cross-sectional design was used. Data was collected by PalliativeQualityCare-Covid19 during fall 2021. 518 respondents from different health care contexts in Sweden participated. Compared with reference values, the results suggested a greater psychosocial work environment regarding meaning of work, social support from colleagues and supervisors, and inferior psychosocial work environment regarding insecurity over working conditions, self-rated health, burnout, and stress. When comparing relocated and non-relocated staff, the results suggested no differences regarding psychosocial work environments. Staff caring for patients with confirmed Covid-19 reported inferior social support from supervisors to those working with patients with non-confirmed Covid-19. Results suggested significant correlations between stress of conscience, organizational self-efficacy, and most dimensions within the psychosocial work environment. Psychosocial work environment and organizational self-efficacy explained 54,2% of the variance in stress of conscience. The results indicate that psychosocial work environment and organizational self-efficacy can be regarded as stressors (risk and health factors) in relation to stress of conscience in health care workers caring for terminally ill patients during the Covid-19-pandemic.
