Abstract [sv]

Tidigare forskning har visat att transpersoner och personer med normbrytande könsuttryck rapporterar sämre psykisk hälsa än cispersoner och personer med normativt könsuttryck. Social ångest har visat sig vara särskilt vanligt bland transpersoner, där minoritetsstress i flera studier lyfts fram som förklaringsmodell. Syftet med föreliggande studie var att i en ung svensk population undersöka skillnader i interpersonella stressorer (som ett proxy-mått på minoritetsstress) och social ångest beroende på könsidentitet eller könsuttryck, samt undersöka kroppsuppfattning och självmedkänsla som möjliga mediatorer mellan interpersonella stressorer och social ångest hos transungdomar samt ungdomar med normbrytande könsuttryck. Studien var av tvärsnittsdesign och en del av det longitudinella forskningsprojektet “Trestadsstudien” vid Örebro Universitet. 3067 deltagare i åldern 17 till 19 år inkluderades. Resultatet visade en signifikant högre utsatthet för interpersonella stressorer och symtom på social ångest bland ungdomar med normbrytande könsuttryck, transungdomar och cistjejer, jämfört med ungdomar med normativt könsuttryck och ciskillar. En signifikant högre förekomst av subklinisk social ångest identifierades bland ungdomar med normbrytande könsuttryck och transungdomar, jämfört med ungdomar med normativt könsuttryck, cistjejer och ciskillar. Kroppsuppfattning och Självmedkänsla medierade sambandet mellan interpersonella stressorer och symtom på social ångest för ungdomar med normbrytande könsuttryck, men inte för gruppen transungdomar. Utifrån studien går det att identifiera en särskild utsatthet för interpersonella stressorer och social ångest inom gruppen transungdomar och ungdomar med normbrytande könsuttryck, samt indikationer om att dessa grupper kan gynnas av att jobba med kroppsuppfattning och självmedkänsla vid behandling av social ångest. Resultaten bör utvärderas och replikeras med longitudinell design, mer tillförlitligt mått på minoritetsstress och större grupp transungdomar, för att kunna uttala sig om kausalitet och medierande samband.

Abstract [en]

Former research show that transgender people and people with norm-breaking gender expression report poorer mental health than cis people and people with normative gender expression. Social anxiety is particularly common among transgender people, and the minority stress model has been highlighted in several studies as an explanatory model. The purpose of the present study was to, in a young swedish population, investigate differences in interpersonal stressors (as a proxy measure of minority stress) and social anxiety depending on gender identity or gender expression, and to examine body image and self-compassion as possible mediators between interpersonal stressors and social anxiety in transgender adolecents and adolecents with norm-breaking gender expression. The study used a cross-sectional design and was part of a longitudinal research project called "Trestadsstudien'' at Örebro University. 3067 participants in the age of 17 to 19 years old were included. The results showed a significantly higher exposure to interpersonal stressors and symtoms of social anxiety among adolescents with norm-breaking gender expression, transgender adolescents and cisgender girls compared to adolescents with normative gender expression and cisgender boys. A significantly higher incidence of subclinical social anxiety was found among adolescents with norm-breaking gender expression and transgender adolescents, compared to adolescents with normative gender expression, cisgender girls and boys. Body image and self-compassion mediated the association between interpersonal stressors and symptoms of social anxiety among adolescents with norm-breaking gender expression, but not within the group of transgender adolescents. The study can identify an higher exposure to interpersonal stressors and social anxiety within the groups of transgender adolescents and adolescents with norm-breaking gender expression, as well as indications that these groups may benefit from working with body image and self-compassion in the treatment of social anxiety. The results should be evaluated and replicated in further studies of longitudinal design, with a more valid measure of minority stress and with a larger group of transgender adolecents.
