Abstract [sv]

Forskning kring schizofrenispektrumsyndrom har visat att det är en allvarlig kronisk psykossjukdom som inte enbart får långtgående konsekvenser för den drabbade individen, utan även för anhöriga. Trots erkända svårigheter har tidigare forskning kring anhörigas situation varit bristfällig i olika avseenden. Forskningsfynd som kopplats till anhöriga har varit begränsade, även inom beforskade områden. Det primära syftet med studien var att undersöka hur olika nivåer av neurokognition, symptomatologi, symptomatisk remission och sjukdomsinsikt samvarierar med rapporterad anhörigbörda. Ett sekundärt syfte var att undersöka variationer över tid. Studien delades in i två steg, där tre tvärsnittliga nedslag kompletterades med en longitudinell ansats i syfte att undersöka variationer över tid. I samarbete med Clinical Long-term Investigation of Psychosis in Sweden (CLIPS) inkluderades 162 patienter; vars närstående besvarat minst ett självskattningsformulär avseende den upplevda anhörigbördan. Vidare insamlades data genom neurokognitiv testning och andra skattningsformulär. Resultatet visade att anhörigbördan, som var relativt låg, samvarierade över tid med patienternas svårighetsgrad av symptom, allmänna psykopatologiska symptom, negativa symtom, symptomatiska remission och sjukdomsinsikt. Sådana samvariationer kunde inte observeras mellan anhörigbörda och patientens positiva symptom samt neurokognitiva förmåga. Slutsatsen var därmed att det finns ett flertal aspekter hos patienten som samvarierar med den anhöriges upplevda börda. Trots det faktum att urvalet inte kan anses vara fullt representativt för de mest betyngda anhöriga eller de sjukaste patienterna, förefaller det tydligt att vara anhörig till någon med en diagnos inom schizofrenispektrum är betingat med negativa konsekvenser. 

Abstract [en]

Research concerning schizophrenia spectrum disorder has shown that it is a serious chronic psychotic illness that not only has far-reaching consequences for the affected individual, but also for relatives. Despite well-known difficulties, previous research regarding the situation for relatives has been deficient in various respects. Findings associated with relatives have been limited, even in researched areas. The primary purpose of the present study was to examine how different levels in neurocognition, symptomatology, symptomatic remission and illness-related insight associates with the relatives' reported burden. A secondary purpose of the study was to examine variations over time. The study was divided into two stages, where three cross-sectional measuring points were supplemented with a longitudinal approach in order to investigate variations over time. In collaboration with Clinical Long-term Investigation of Psychosis in Sweden (CLIPS) a total of 162 patients were included, whose relatives had answered at least one self-assessment form regarding the perceived burden of care. Data was also retrieved through neurocognitive testing and other assessment forms. The results showed that the relatives’ quite low reported burden correlated over time with the patients’ severity of symptoms, general psychopathological symptoms, negative symptoms, symptomatic remission and illness-related insight. Such correlations could not be obtained between the relatives’ reported burden and the patients’ positive symptoms and neurocognitive ability. Thus, the conclusion drawn was that there are several aspects of the patient that is associated with the relatives’ perceived burden of care. Despite the fact that the sample cannot fully be considered representative of the most burdened relatives or the most unwell patients, it remains clear that being related to someone with a diagnosis within the schizophrenia spectrum is conditioned by negative consequences.
