Abstract [sv]

Långvarig smärta orsakar stort lidande för många i Sverige och är en av de vanligaste orsakerna till långtidssjukskrivning. Många drabbade upplever kognitiva svårigheter, bland annat minnesnedsättningar, men det behövs mer precis kunskap om nedsättningarnas karaktär. Syftet med föreliggande examensarbete var att studera eventuella skillnader i episodiskt minne hos en grupp med långvarig smärta (n=29) jämfört med en frisk kontrollgrupp (n=40). Objektiv testprestation undersöktes med Buschkes selektiva minnesprocedur [Buschke] och subjektivt upplevda nedsättningar i vardagen skattades med formuläret Prospective and Retrospecive Memory Questionnaire [PRMQ]. Smärtgruppen presterade lika bra som kontrollgruppen på Buschke men skattade mer episodiska minnesnedsättningar i vardagen på PRMQ. Det gällde i synnerhet prospektivt minne, men den skillnaden mellan grupperna skulle kunna förklaras av en högre nivå av depressiva symtom hos smärtgruppen. Det går delvis i linje med tidigare forskning. I klinisk verksamhet bör man alltså vara särskilt uppmärksam på samtidig förekomst av långvarig smärta och depression.

Abstract [en]

Chronic pain causes great suffering for many people in Sweden and is a common cause of longterm sick leave. Many patients with chronic pain experience cognitive impairments, including memory failures, but there is still a need for more precise knowledge regarding these impairments. The purpose of this study was to assess possible differences in episodic memory in a group with chronic pain (n=29) compared to healthy controls (n=40). We used the Buschke Selective Reminding Test [Buschke] to assess objective performance, along with the Prospective and Retrospective Memory Questionnaire [PRMQ] to measure self-assessed experience. Both groups performed equally on Buschke. However the results on PRMQ showed a higher level of everyday episodic memory failures in the group with chronic pain. The difference was especially large when it came to prospective memory failures, but higher levels of depressive symptoms might explain the difference between groups. This possibility is partially supported by previous research. One implication is that the combination of chronic pain and depression demands extra attention in clinical settings.
